Job summary

We are excited to be looking for 2 newly qualified or experienced Physician Associates to join our multidisciplinary team. We have vacancies at both our Shoreham Health Centre and Northbourne Medical Centre sites. We are keen to increase our PA workforce and you will be supported by GPs, nurses, HCAs, other ARRS-employed clinicians and our experienced PA Ambassador. You will be expected to participate in all aspects of patient care through knowledge gained via your training with a view to expanding your career.

Main duties of the job

To work within your scope of clinical practice as part of a multidisciplinary team and be the first contact of care for patients. You will provide care from the initial history taking, diagnosis, treatment and management of patients under the supervision of a GP. You will support the delivery of policy and procedures and meet the needs of our patient population. You will demonstrate safe decision making and expert care for patients within the practice.

Consultations will be face to face, telephone and home visits, with a mixture of acute and chronic condition management. There will also be a need for analysing test results and following up patients, as appropriate. There will be an opportunity to develop a specialist interest in the future.

You will be expected to work to the interim GMC standards for Physician Associates and be professional at all times, with a caring attitude towards patients. Those who are new to primary care within the first 3 years of qualification, we would be keen to commence you on the PA Preceptorship Scheme.

Mentorship and supervision would be dependent on the post holders skills, experience and knowledge. We have started supervising PA students from Brighton and Sussex Medical School from this year and we are very keen to continue supporting PA development in the Sussex area.

We will develop a system to facilitate a smooth process for the management of signing off X-ray requests, MED3 and prescriptions (already occurring at one of our practices)

You will be invited to attend monthly evening education sessions and Practice Learning Events as appropriate

You will be able to regularly attend PA KSS virtual teaching and PA peer support sessions with the other PA in the PCN

Job description

Job responsibilities

The Physician Associate has the following key responsibilities in delivering health services at Adur Health Partnership. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Provide first point of contact care for patients presenting with undifferentiated, undiagnosed problems by utilising history-taking, physical examinations and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient (and their carers where applicable)

b. Following appropriate training, to support the management of patients conditions by offering clinics including (but not limited to) family planning, cervical screening, baby checks, COPD, asthma, diabetes and anticoagulation

c. Provide health/disease promotion and prevention advice, alongside analysing and actioning diagnostic test results

d. Develop integrated patient-centred care through appropriate working with the wider primary care multi-disciplinary team and social care networks

e. Utilise clinical guidelines and promote evidence-based practice and partake in clinical audits, significant event reviews and other research and analysis tasks

f. Participate in duty rotas; undertaking face-to-face, telephone and online consultations for emergency or routine problems as determined by the organisation, including the management of patients with long-term conditions

g. Develop and agree a personal development plan (PDP) utilising a reflective approach to practice, operating under appropriate clinical supervision

h. Receive referrals and directed patients from triage services and other clinicians

i. Manage a caseload of patients and work with care institutions

j. Plan, implement and evaluate treatment/interventions for patients with acute and chronic physical and psychological conditions

k. Utilise as needed the full range of non-drug therapies for all patients, consider pharmacological interventions and make recommendations to prescribers in accordance with evidence-based practice and national, local and practice protocols

l. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

m. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care

n. Review the latest guidance ensuring the organisation conforms to NICE, CQC etc.

o. Provide targeted support and proactive reviews for vulnerable, complex patients and those at risk of admission and re-admission to secondary care

p. Be responsive to the changing needs of patients and service provision in terms of scope and nature of clinical practice

q. Prioritise, organise and manage own workload in a manner that maintains and promotes quality

r. Undertake clinical responsibilities as required by the organisation within scope of practice defined by RCP/GMC

s. Identify and be proactive in own training needs and development and as agreed with clinical mentor

t. Recognise and work within own competence and professional code of conduct as regulated by the GMC

u. Produce accurate, contemporaneous and complete records of patient consultations, consistent with legislation, policies and procedures

v. Support in the delivery of enhanced services and other service requirements on behalf of the organisation

w. Support virtual and remote models of consultation and support including e-consultations, telehealth and telemedicine

x. Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near miss events

y. Develop yourself and the role through participation in clinical supervision, training and service redesign activities

z. Contribute to a patient safety culture through the reporting and investigation of significant events and patient safety incidents and to undertake proactive measures to improve patient safety

aa. Actively signpost patients to the correct healthcare professional

bb. Undertake all mandatory training and induction programmes

cc. Contribute to and embrace the spectrum of clinical governance

dd. Attend a formal appraisal with your manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed

ee. Contribute to public health campaigns (e.g., COVID-19 and flu clinics) through advice or direct care

ff. Maintain a clean, tidy, effective working area

Secondary Responsibilities

In addition to the primary key responsibilities, the Physician Associate may be requested to:

a. Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives

b. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner

c. Duties may vary from time to time without changing the general character of the post or the level of responsibility.

d. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

Professional Development

Demonstrate an understanding of current educational policies relevant to working areas of practice and keeps up to date with relevant clinical practice.

Maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process.

Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.

Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.

Work with your line manager to access regular clinical supervision, to enable you to deal effectively with the difficult issues that people present.

Review yearly progress and develop clear plans to achieve results within priorities set by others. Participate in the delivery of formal education programmes.

Demonstrate an understanding of current educational policies relevant to working areas of practice and keep up to date with relevant clinical practice.

Work towards developing competence and achieving agreed goals to be eligible to progress to a PCN Clinical Pharmacist post.
