Job summary
The SPWLW works alongside a team within general practice and within the PCN and empowers people to take control of their health and well-being.
The post-holder will be an integral part of the PCN team, working across the PCN, as well as part of a wider community groups and their multidisciplinary teams. A SPWLW supports existing groups to be accessible and sustainable and helps people to start new community groups, working collaboratively with all local partners.
A referral to a non-medical link worker is designed to support patients to be able to take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.
Social prescribing can help to strengthen community resilience and personal resilience while reducing health inequalities by addressing the wider determinants of health such as debt, poor housing and physical inactivity by increasing peoples active involvement with their local communities.
This role can be particularly beneficial to patients with long-term conditions, those with mental health issues and those who are lonely or isolated or who have complex social needs which affect their wellbeing.
Main duties of the job
Provide care within own competence and seek advice or refer to colleagues when appropriate, ensuring that practice is effective, relevant, evidence-based and of a high standard.
Develop knowledge of local services to enable patients to access a range of services to meet their needs and ensure they are engaged and connected with their local community and other organisations and groups to make best use of resources.
Take referrals from the local network(s) and work with the referrer and the patient to plan a holistic based approach to care to include a personalised care plan.

About us
The award-winning Selby Town PCN consists of four practices with a combined population of approximately 50,000 patients, covering the large geographical area surrounding Selby and Escrick. The successful candidate(s) will be employed by Selby Prime Care Ltd who are contracted to provide PCN activities to the practices in the locality.
We are looking to recruit 3 x whole time equivalent posts to support patients & staff across the four practices in the PCN: Beech Tree Surgery, Escrick Surgery, Posterngate Surgery and Scott Road Medical Centre. The PCN has a Clinical Director, two Clinical Lead GPs and is managed by the PCN Operations Manager & HR Lead. We work in increasingly collaborative partnerships across a number of workstreams, which include important priority areas of mental health, frailty, urgent care and health inequalities & population health. Our team consists of a diverse multi-professional workforce which includes pharmacists, MSK practitioners, mental health practitioners, care coordinators, a frailty therapy team, an urgent care coordinator, a business intelligence analyst/digital lead, a transformational lead and an urgent care team of health care professionals.
