Job summary

We are looking for an Advanced Clinical Practitioner (ACP) who can work alongside partners and team members to help deliver urgent same-day and next-day general practice to our population. We are working with our local Urgent Treatment Centre and Acute Trust to redesign the delivery of urgent care in our locality and are recruiting suitably qualified staff who can contribute to the next phase of its development.

This is an exciting opportunity for an innovative and motivated ACP to be part of this project as we move towards an integrated urgent care solution for the locality. It is envisaged that one of the local practices will host this role temporarily, with the intention of moving to a permanent base within Selby Hospital, co-located with the existing urgent treatment centre.

Main duties of the job

The successful candidate should be an experienced practitioner who, acting within their professional boundaries and competencies, will provide care to the presenting patient, from initial clinical assessment, through diagnosis, treatment, and an evaluation of care. They will demonstrate safe and effective clinical decision making and critical thinking. They will be a caring and compassionate clinician who puts the needs of their patient at the heart of everything they do.

About us

The award-winning Selby Town PCN consists of four practices with a combined population of approximately 50,000 patients, covering the large geographical area surrounding Selby and Escrick. The successful candidate) will be employed by Selby Prime Care Ltd who are contracted to provide PCN activities to the practices in the locality.

This is a post supporting patients & staff across the four practices in the PCN: Beech Tree Surgery, Escrick Surgery, Posterngate Surgery and Scott Road Medical Centre. The PCN has a Clinical Director, a Clinical Lead GP and is managed by the PCN Operations Manager. We work in increasingly collaborative partnerships across several workstreams, which include important priority areas of mental health, frailty, urgent care and health inequity. Our team consists of a diverse multi-professional workforce which includes pharmacists, MSK practitioners, mental health workers, care coordinators, social prescribing link workers, a frailty therapy team, an urgent care coordinator, a business intelligence analyst/digital lead, a transformational lead and an urgent care team of health care professionals.

Job responsibilities

Assess, diagnose, and treat patients presenting with an undifferentiated diagnosis.

Consider the holistic biopsychosocial needs of patients and plan clinical care accordingly.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based national and local protocols, and within scope of practice.

Recognise and work within own competence and professional code of conduct as regulated by theNMC or HCPC.

Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures.

Deliver care according to NSF, NICE guidelines, and evidence-based care.

Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation.

Evaluate patients response to health care provision and the effectiveness of care.

Support and participate in shared learning across the Primary Care Network

Use a structured framework (e.g., root-cause analysis) to manage, review and identify learning from patient complaints, clinical incidents, and near-miss events.

Act as a clinical leader in the delivery of services ensuring that the needs of the patient are a priority.

Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model.

Support staff development to maximise potential.

Contribute to the development of local guidelines, protocols and standards.

Undertake mentorship for more junior staff, assessing competence against set standards.

Disseminate learning and information gained to other team members to share good practice and inform others about current and future developments (e.g. courses and conferences)

Assess own learning needs and undertake learning as appropriate.

Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.
