Job summary
The SS9-North Primary Care Network (PCN) Care Coordinator will
play a key role in supporting the efficient operation of PCN services and
improving patient care delivery. This position involves coordinating clinical
rotas, maintaining accurate records and systems, supporting multidisciplinary
teams, and facilitating communication between PCN practices, community
services, and partner organisations.
The Care Coordinator will ensure that clinical and
administrative processes run smoothly, contributing to the effective delivery
of Enhanced Access and other Local / Directed Enhanced Services (LES/DES)
Projects, Integrated Neighbourhood Teams (INT) collaboration, and general
operational support for the PCN.
Main duties of the job
The Care Coordinator will play a key role in supporting the Primary Care Network (PCN) Management Team in the delivery of efficient, effective, and high-quality services across the network. This role will provide vital operational, digital, and administrative support, helping to ensure the smooth running of PCN projects, programmes, and day-to-day activities.
The postholder will work closely with PCN leadership, practice teams, and external partners to coordinate activities, track progress, and maintain clear communication across the network. They will also contribute to the implementation of digital tools and data systems that support improved patient care, workforce efficiency, and service delivery.

Operational and Administrative Support
- Coordinate and maintain PCN staff rotas, including weekend and service-specific schedules.
- Manage updates and allocation of staff shifts in collaboration with clinicians and managers.
- Maintain oversight of daily operations, providing responsive support to staff and resolving queries promptly.
- Record, distribute, and follow up on meeting minutes, ensuring key actions are completed.
- Oversee ordering of clinical equipment and consumables for PCN services (e.g. minor ops, phlebotomy).
- Support compliance with policies and procedures through regular review and updates.
Service Coordination and Liaison
- Liaise with Integrated Neighbourhood Teams (INT) to oversee shared rotas and service provision.
- Support communication between the PCN and partner organisations to ensure seamless patient care pathways.
- Coordinate submissions and reporting for PCN requirements, such as NWRS (National Workforce Reporting System) and other statutory returns.
- Assist with monitoring and delivery of LES and DES requirements.
Governance and Policy Monitoring
- Maintain and update PCN policies and procedures in collaboration with the management team.
- Ensure that relevant documents are reviewed regularly and meet compliance standards.
- Contribute to audits and reporting as required to support PCN quality improvement and assurance.
