Job summary
Shore Medical is seeking an experienced Clinical Pharmacist to work across all Shore Medical practices.
The role focuses on ensuring the delivery of safe, effective and efficient systems for repeat
prescribing, medicines optimisation, reducing medicines waste and maximising
patient outcomes. You will be part of a larger organisation and supported by a
GP led Prescribing Team.
Job Type: Permanent- Full Time
Hours: 37.5 hours across four days (Monday, Tuesday, Thursday & Friday)
Main duties of the job
The key aims for the Clinical Pharmacist include
- To reduce the medicines related workload on
doctors
- Increase safety and quality of prescribed
medicines
- Provide support on medicines related issues to
the whole practice team
- Be a point of contact for patients regarding
medicines queries
They should support the following clinical work:
- Clinical Medication Review
- Chronic Disease Management (for example,
managing hypertension/Type 2 Diabetes/Asthma/COPD patients, polypharmacy
review)
- Medicines reconciliation on transfer between
care providers

- -Minimum of 2 years post-qualification experience
- -Awareness and understanding of professional accountability, GPhC standards and other local initiatives
- -Evidence of continuing professional development
- -Experience of working in a multidisciplinary environment with a wide range of individuals at different levels.
- -Experience of developing relationships with external agencies and organisations
- -Good all-round clinical knowledge and practical clinical skills
Desirable
- -Experience of Chronic Disease management
- -Experience of working in routine Primary Care
Qualifications
Essential
- -Masters Degree in Pharmacy (MPharm) or equivalent
- -Registered as a pharmacist with the General Pharmaceutical Council (GPhC)
- -Independent Prescriber with GPhC (or willing to work towards the qualification)

- -Post Grad Diploma in Clinical Pharmacy and or NHS England GP Practice Pathway accreditation (PCPEP)
- -Specialist knowledge acquired through post- graduate diploma level or equivalent training / experience in relevant clinical subject
