Health Care Assistant- Frailty

Job summary

Shore Medical is recruiting an experienced Health Care Assistant- Frailty to be part of the Primary Health Care Team providing high quality and forward-thinking nursing care to meet the holistic healthcare needs of the Practice population, working to agreed Practice standards and protocols for the delivery of patient care.

This role is based at our Parkstone Tower Practice in Poole and is a full time permanent position.

Main duties of the job

Key Responsibilities

1.Works under indirect supervision of the Practice Nurses and Nursing Lead

2.Is accountable to the Nursing Lead and the Primary Care Network for their actions in undertaking delegated work.

3.You will be responsible for attending to patients who wish to be seen by a health care professional within the Patients Home, providing high quality evidence-based nursing services to the population of Shore Medical.

4.Provides support to other members of the primary health care team and contributes towards the co-ordination of team activities and the development of the Practice as a whole.

5.Refers to more senior members of the Primary Health Care team as appropriate.

Clinical Responsibilities

The Health Care Assistant undertakes procedures that are agreed by the Head of Nursing which are likely to include:

1.Venepuncture

2.BP checks, height, weight, BMI, urinalysis, ECG

3.NHS health checks

4.Long term condition/Chronic disease health checks

5.Flu & Covid vaccination (following approved training and competence checks

6.Diabetic Foot Checks (following approved training and competence checks)

7.Dressings (following approved training and competence checks)
