Job summary

An exciting opportunity has arisen for a Clinical Pharmacist to undertake a pioneering role within primary care.

Working as part of a team which currently comprises of three Clinical Pharmacists and a Pharmacy Technician. The PCN is open to applications from newly qualified Pharmacists.

You will work closely with all PCN and practice staff.

Main duties of the job

To work as part of a multi-disciplinary team in a patient-facing role.

Provide clinical leadership on medicines management and quality improvement as well as managing some aspects of the quality and outcomes framework and enhanced services.

Primary Duties and Areas of Responsibility includes but not limited to: Long-term condition Clinics, Structured Medication Review, medicines support, medicine information to practice staff and patients, unplanned hospital admissions reviews, management of medicines at discharge from hospital, repeat prescribing, risk stratification, service development, information management, medicines quality improvement, medicines safety, implementation of local and national guidelines and formulary recommendations.

Previous experience of working in a GP practice is desirable but not essential. Applicants will need to be computer literate, confident in working independently, able to prioritise workload and want to work in a multi-disciplinary environment.

  • -Mandatory registration with General Pharmaceutical Council
  • -Membership of the Royal Pharmaceutical Society
  • -A member of or working towards Faculty membership of the Royal Pharmaceutical Society
  • -Professional Indemnity Insurance is required and will be reviewed on an annual basis.
  • -Masters degree in pharmacy (MPharm)
