Acute Home Visiting Nurse (prescribing)

Job summary

We are offering an exciting and fast paced role at West Leeds Primary Care Network (WLPCN) for a prescribing nurse or nurse practitioner to join our highly skilled and experienced home visiting team. This is for the Acute Visiting element of the service; we undertake to see as many of the acutely unwell patients across our member practices as we can in order to alleviate pressure on the GPs within our practices. It is a highly valued service and as such we need a really robust and experienced NMC registered clinician to undertake this work. Main duties of the job

You will see acutely unwell patients that have requested an urgent same day home visit via their GP surgery. This role will suit someone with prescribing and minor illness experience. Please see uploaded job description document for more details about the kind of clinical work this will involve

About us

West Leeds Primary Care Network provides enhanced clinical services for six GP surgeries across the Pudsey and Bramley area with a patient population of approximately 70k.

Job description

Job responsibilities

This role will be mainly reactive work for which we need an experienced Independent Nurse Prescriber with a good breadth of general healthcare experience who can undertake acute case home visiting. You will be part of a very experienced team and there will be plenty of debrief opportunities with other clinicians, including prescribing Advanced Clinical Practitioners within your own team. You will be assigned a clinical supervisor to help with your continued professional development and training opportunities will be available from time to time. Your responsibilities will include the assessment and treatment of acutely unwell patients who will be booked on to your ledger after having contacted their own GP surgery. You will have a fixed number of home visits per day once you have completed your induction period.

You will have the opportunity to work collaboratively with other specialists / clinical disciplines within our organisation in a bid to provide truly holistic care, improving patient outcomes.

You will be responsible for assessing acute conditions in the patients residence, providing treatment and safety netting as well as actioning appropriate onward referrals (such as the Virtual Home Ward).

You will have access to a GP on call for immediate advice and guidance should you need it.

You will manage your own visits based on prioritising the needs of the patient. You may be responsible for organising hospital admission should it be needed.

You will be expected to work independently in the patients residence and take responsibility for your own clinical decisions.

You will have access to multiple clinical specialists who can advise on a care plan following your clinical assessment.

You will be exposed to a variety of acute conditions such as UTIs, chest infections, oedema, infected wounds, migraines etc. as well as exacerbation of chronic illness. Experience in these areas will be essential

You will maintain accurate clinical records of all patient consultations and related work

The provision of this service is vital to our member practices and frees up practice GP time while we fulfil these visits for them
