Job Overview
To improve patients' health outcomes and the efficiency of
the primary care network (PCN) pharmacy team by providing direct, accessible
and timely medicines expertise as part of a multi-disciplinary team. To pro-actively complete workloads relating
to medicines optimisation issues from other clinical staff to improve patient
care, safety and outcomes.
The Clinical Pharmacist must be registered with the General
Pharmaceutical Council (GPhC) and enroll in, or already be qualified from, an
approved 18-month training pathway or equivalent that equips the Clinical
Pharmacist to be able to practice and prescribe safely and effectively in a
primary care setting, for example, the CPPE Clinical Pharmacist training
pathways.
The summary of the role includes:
a. work as part of a multi-disciplinary team in a
patient facing role to clinically assess and treat patients using their expert
knowledge of medicines for specific disease areas;
b. be a prescriber, or completing training to become a
prescriber on completion of the 18-month training pathway, and work with and
alongside the general practice team;
c. be responsible for the care management of patients
with chronic diseases and undertake clinical medication reviews to proactively
manage people with complex polypharmacy, especially older people, people in
care homes, those with multiple co-morbidities (in particular frailty, COPD and
asthma) and people with learning disabilities or autism (through STOMP Stop
Over Medication Programme)
d. provide specialist expertise in the use of medicines
whilst helping to address both the public health and social care needs of
patients at the PCNs practices and to help in tackling inequalities;
e. provide leadership on person-centered medicines
optimisation (including ensuring prescribers in the practice conserve
antibiotics in line with local antimicrobial stewardship guidance) and quality
improvement, whilst contributing to the quality and outcomes framework and
enhanced services;
f. through structured medication reviews, support
patients to take their medications to get the best from them, reduce waste and
promote selfcare;
g. have a leadership role
in supporting further integration of general practice with the wider healthcare
teams (including community and hospital pharmacy) to help improve patient
outcomes, ensure better access to healthcare and help manage general practice
workload;
h. develop relationships
and work closely with other pharmacy professionals across PCNs and the wider
health and social care system;
i. take a central role in the clinical aspects of shared
care protocols, clinical research with medicines, liaison with specialist
pharmacists (including mental health and reduction of inappropriate
antipsychotic use in people with learning disabilities), liaison with community
pharmacists and anticoagulation; and
j. be part of a professional clinical network and have
access to appropriate clinical supervision. Each clinical pharmacist must receive
a minimum of one supervision session per month by a senior clinical / advanced
practice pharmacist;
k. the senior clinical / advanced
practice pharmacist must receive a minimum of one supervision session every
three months by a GP clinical supervisor;
l. each clinical pharmacist/senior
clinical pharmacist/advanced practice pharmacist will have access to an
assigned GP clinical supervisor for support and development;
m. a ratio of one senior clinical /
advanced practice pharmacist to no more than five junior clinical pharmacists,
with appropriate peer support and supervision in place.
k. undertakes clinical audits as agreed with the Prescribing
Lead
l.receive and
resolve medicine queries from patients over the telephone and/or face to face.

Responsibilities underpinning the role
To develop and facilitate a good working relationship with all
local providers of healthcare.
To work closely with the Prescribing Lead and
Practice Managers and agree job objectives and timetable
To record personally generated
information and maintain a database of information relating to the work done in
the practice(s). Personally generated information includes information and
records relating to audit and clinical work undertaken by the post holder,
reference notes relating to clinical/technical information.
Maintain records of all work done in
the practice for which the post holder is accountable through the correct use
of the clinical system.
To maintain registration as a
pharmacist and comply with appropriate professional codes.
As appropriate to the post, to maintain
and develop professional competence and expertise, keep up to date with
medical/therapeutic evidence and opinion, local and national service
specifications, legislation and policy developments,
To agree objectives and a personal
development plan and participate in the appraisal process.
To attend local, regional and national
meetings of relevance as required by the practice.
To undertake any other duties
commensurate with the post holders grade as agreed with the post holders line
manager.
