Job summary

The Clinical Pharmacist will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the Practice.

The Clinical Pharmacist will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

Main duties of the job

Review medicine needs for patients with single or multiple medical problems where medicines optimisation is required.

Undertake clinical medication reviews and produce recommendations for GPs and nurse team on prescribing and monitoring

Undertake care home medication reviews and work with care home staff to improve safety of medicines ordering and administration

Answer relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines. Provide follow up for patients to monitor the effect of any changes

Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge

Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g. those with medicine compliance aids or those in care homes)

About us

The Clinical Pharmacist will work as part of a multi-disciplinary team in a patient-contact role, either remotely or face-to-face in the Wisbech PCN. The PCN has approximately 50,000 patients and comprises of 4 practices North Brink Practice, Trinity Surgery, Clarkson Surgery and Parson Drove Surgery.

Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets)

Analyse, interpret and present medicines data to highlight issues and risks to support decision- making

Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement the changes in conjunction with the practice team

Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance

Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.

Experience

Essential

  • Minimum 2 years post registration experience
  • Sound clinical and therapeutic knowledge and of evidence based health care
  • Excellent written, electronic and verbal communication skills
  • Able to communicate complex and sensitive information to patients or wider group audiences, using a variety of methods of communication

Desirable

  • An understanding of General Practice, including the different roles within Practice teams and the delivery of services.
  • An understanding of General Practice prescribing, factors influencing this, rational prescribing and strategies for improving quality of prescribing.
  • Experience of working within Primary Care Team and /or community health team

Other requirements

  • Ability to travel and work across multiple sites with use of own car (business use included on insurance policy)
  • Willingness and ability to work remotely and from home
  • Full clean UK driving licence and including business insurance cover
