Working Hours: 22.5 37.5 hours per week with flexibility

Reports to: Clinical Director/PCN Board

Employed by: Whitewater Loddon PCN Hosting Practice

Salary: £46,820 full time (pro-rata for reduced hours)

An exciting opportunity has presented within our network for a Digital Transformation Lead/PCN Manager. Approximately 80% of the role will be as DTL with the remaining 20% as PCN Manager.

Reporting to the Clinical Director, the successful candidate will work with the PCN Board to implement the PCN key objectives, whilst managing high quality projects, liaising with our stakeholders and overseeing the governance, HR and administration of the PCN.

The Digital Transformation Lead/PCN Manager will provide leadership across the PCN and focus on the following key responsibilities-

Main duties of the job

  • Develop and implement the Digital & IT Transformation Plan with the intention of ensuring that the PCN and its member practices consistently utilise the latest technology to improve patient services; maximise clinical efficiency; enhance internal communications and support improved information management.
  • Project management - support project management across the PCN to deliver workstreams.
  • IM&T deliver an IT strategy fit for 21st Century, working in partnership with the Practices, ICB and third-party providers.
  • Strategy and Leadership - operational leadership and line management to ARRS PCN staff.
  • PCN Workforce - leading the team and developing future workforce plans.
  • Governance - ensure governance of the PCN both a clinical and corporate perspective.
  • Communication & Relationships - maintain robust communication across PCN staff, PCN practices and external organisations.
  • Finance support and oversee the daily management of PCN finances to ensure value for money for NHS resources and financial balance.
  • IM&T deliver an IT strategy fit for 21st Century, working in partnership with the Practices, ICB and third-party providers.
  • Develop good working relationships with the ICB and other IT & Digital teams to identify opportunities for support and development.

Whitewater Loddon PCN is an aspiring and forward thinking PCN based in the Basingstoke area and have a patient population of just under 42,000 patients. We work closely with our member practices, Chineham Medical Practice, Whitewater Health (Hook and Hartley Wintney) and Clift Surgery in Bramley.

Whitewater Loddon PCN is based in the north of Hampshire and is commissioned by Hants and IOW ICB.We work with other organisations and communities to promote and drive the development of services that best reflect the needs of our local population.

Other members of the PCN workforce includes a team of Clinical Pharmacists; First Contact Physiotherapists; Social Prescribers; Mental Health & Wellbeing Practitioners; Health & Wellbeing Coach; Frailty Teams and Care Co-ordinators.

Objective of the Role:

The objective of the DTL/PCN Manager is to provide strategic leadership and overseeing the management for Whitewater Loddon PCN, ensuring the delivery of high-quality healthcare services and the achievement of PCN objectives. This role encompasses the following key objectives:

Lead and develop a strategic service and business plan, including a robust digital infrastructure for the PCN, underpinned by a quality improvement methodology.

Drive integrated working and reduce duplication across practices within the PCN.

Reduce variation between practices, ensuring consistent and high-quality care delivery.

Provide data-driven decision-making, encompassing demand, capacity, utilisation data, staff, and patient experience.

Build staff skills and capabilities, to also include digital advancements and healthcare best practices.

Support Practices with strategic planning to achieve the DES Contracts requirements.

Take into consideration the digital maturity of the PCN and the population's health needs and health inequalities within the community. Improve digital accessibility for patients.

To ensure all the contractual obligations of the PCN are properly structured in order that all reports, metrics and technical reporting (both internally and externally) are robust.

Main Responsibilities:

1. Leadership and Management:

Provide strategic and operational leadership to the PCN, aligning objectives with the PCN Board's vision, strategy, and priorities.

Lead and support the Digital and Transformation Programme, and associated staff in developing and implementing the strategic plan for the digital infrastructure.

Manage and monitor the performance of the PCN, ensuring the delivery of high-quality services and achievement of targets.

Foster a culture of collaboration, innovation, and continuous improvement within the PCN.

2. Operational Management:

Supporting the PCN member Practice management teams continuous training and development.

Work closely with practice managers, clinical leads, and other stakeholders to optimise workflows and enhance integrated working, also adopting digital tools where appropriate.

Monitor and evaluate the performance of practices within the PCN, identifying areas for improvement and support.

Ensure compliance with relevant regulations, policies, and guidelines, and the appropriate stewardship of any public finances attributed to the PCN, in conjunction with the external accountants.

3. Digital Management:

Develop and maintain a deep understanding of the PCN's digital infrastructure and business intelligence tools.

Procure and manage digital tools aligned with PCN strategy.

Oversee the management of all digital projects, evaluating their effectiveness.

Ensure initiatives have clinical champions and practice management support.

Lead the development and implementation of digital transformation bids.

Ensure initiatives are scoped and underpinned by a quality improvement methodology.

Stay updated with national and local PCN guidance as they relate to digital transformation.

4. Financial Management:

Alongside the Clinical Director and Financial Manager oversee the financial resources of the PCN, overseeing budgeting, financial planning, and reporting.

Monitor expenditure, identify cost-saving opportunities, and ensure financial sustainability.

Collaborate with the PCN Board on financial decisions and funding applications, and appropriate levels of financial governance.

5. Stakeholder Engagement and Partnership Working:

Build and maintain strong relationships with internal and external stakeholders, including practice staff, private providers, local primary care teams, neighbouring PCNs, and key partners.

Collaborate with stakeholders to develop and implement joint initiatives and share best practices.

Represent the PCN in meetings, conferences, and forums, advocating for the PCN's interests and promoting its objectives.

6. Governance and Accountability:

Report to the PCN Board, providing regular updates on all performance areas associated with PCN business, its achievements, and challenges of the PCN.

Ensure effective governance and risk management processes are in place.

Maintain accurate documentation related to finances, standard operating procedures, contracts, contacts, and decision-making.
