About the Service

The Urgent Care Hub provides same day access for patients requiring urgent face-to-face assessment following virtual clinical triage by their home practice. The service is designed to provide additional same-day capacity to our four local general practice surgeries and improve patient flow by offering rapid, high quality clinical input in a centralised setting. You will be supported by an experienced team and clear clinical pathways. In addition, you will have dedicated slots to provide clinical advice and support to colleagues from the adjacent ACP-led urgent treatment centre.

Opportunities could also exist for the right candidate to support local practices in other aspects of their work such as the Frailty Proactive Care Team, general clinical duties, or in teaching and training.

Clinical Care

Provide timely assessment, diagnosis, and management of patients presenting with urgent or same day care needs

Manage acute presentations, minor illness, and undifferentiated symptoms

Undertake face to face, telephone, or remote consultations as required

Make safe and appropriate onward referrals to secondary care or community services

Provide clear safety netting advice and follow up planning

Support clinical decision making within the multidisciplinary team

Teamwork & Communication

Work collaboratively with the clinical and support teams in the four practices.

Work collaboratively with ANPs, ACPs, nurses, and administrative staff within the UTC and PCN Urgent Care Hub

Contribute to a positive, supportive team culture within the PCN, practices and urgent care hub.

Communicate effectively with patients, carers, and colleagues

Maintain accurate, contemporaneous clinical records

Whitby Coast and Moors PCN comprises four GP practices in Whitby and the surrounding area: Whitby Group Practice, Esk Valley Medical Practice, Staithes Surgery, and Sleights and Sandsend Medical Practice. Our population faces particular challenges relating to higher than average chronic disease, coastal deprivation, and rural isolation.

The purpose of this role is to deliver efficient and effective clinical care to patients attending the Urgent Care Hub.To work as an autonomous practitioner, responsible for the provision of medical services to the PCN population, delivering an excellent standard of clinical care whilst complying with the GMS contract.

Furthermore, the post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the PCN, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.

You will provide NHS general medical services to registered patients on the PCN population.

You will be required to provide all core primary care services as defined by the local CCG and NHS England requirements, including:

Clinical care to patients; consultation sessions, telephone triage, online consultation.

Investigation and examination, treatment and diagnosis or referral for medical problems presented by patients.

Provide preventative health care and advice to patients as required.

Adhere to the practice prescribing formulary, local and national guidelines.

Action test results, medical reports and examinations as necessary.

Complete necessary clinical administration for the role.

Work as part of a team to deliver QOF, enhanced services and KPI targets

Engage with NHS annual strengthened appraisal, PDP production, GMC revalidation and ongoing CPD.

Comply with practice policy or guidelines, including data protection, health and safety, QOF and prescribing confidentiality.
