Job summary

An exciting opportunity has arisen for Project Officer within Whitby Coast and Moors Primary Care Network. We are seeking a proactive and organised Project Officer, whose primary role will be to assist the Digital and Transformation PCN Manager with the PCN pilot and transformational projects at both PCN and practice level.

Whitby Coast and Moors Primary Care Network is evolving and entering an exciting period of growth which has enabled the recruitment of an experienced Project Officer.

We are a forward thinking and evolving PCN who strive to create a difference in patients lives and keep patients living well in their homes for longer.

This key role will be embedded in the PCN Pilot and Transformation Team within Whitby Coast and Moors Primary Care Network, which supports system wide collaborative working to deliver multiple projects in work streams.

Main duties of the job

The primary focus of the post holder will be to support the planning, development, and implementation of a range of projects that fall within the scope of the PCN, including the PCN pilot.

The post holder will be required to engage a range of key partners and stakeholders, and lead on joint working across health and social care. They will need to work closely with colleagues within the PCN, Practices and ICB, councils, and the voluntary and community sector. They will also work alongside our primary care locality groups to support them in meeting their priorities around prevention and self-care.

About us

Whitby Coast and Moors PCN was founded in 2020 and consisted of 5 GP surgeries namely, Whitby Group Practice, Sleights and Sandsend Medical Practice, Danby Surgery, Egton Surgery and Staithes Surgery with a population in the region of 25,500 patients. Since inception the patient population has grown to approximately 27,010 and Danby and Egton surgeries merged to become Esk Valley Medical Practice.

Whitby is relatively small PCN with a large geographical area of approximately 150 square miles. Whitby PCN is both rural and coastal, having one "A" road in and out in both directions and a high percentage of elderly patients.

Key Responsibilities:

Project Coordinator under delegation from Digital and Transformation PCN Manager

To support the Digitan and Transformation PCN Manager with day to day running on the PCN.

Providing leadership and guidance to all staff ensuring that they adhere to policy and procedure at all times.

To investigate and resolve problems in the day-to-day use of computers, printers, and other IT related equipment.

Leading the management of the clinical system, ensuring IT security and IG compliance at all times, and responding to and resolving all local IT issues.

Implementing systems to ensure compliance with CQC regulations and standards.

To Investigate new and emerging technologies, drafting proposals, and presenting ideas at the PCN board and pilot steering group meetings.

To undertake and support project management processes for the PCN Pilot.

Undertake information/project analysis/evaluation tasks.

Contribute to information and records management.

Liaise with Business Intelligence colleagues across the PCN Network to request, analyse and report on healthcare related data.

Maintain data collection systems and processes for effective use by the team to support project/programme meeting discussions and priority planning.

Present project updates at Pilot/Programme/Project meetings.

Support development and maintenance of project documentation including project plans, data collection, risk, and issues logs.

Support with maintaining governance structures of projects via internal and external processes including risk escalation or approval processes.

Support with audit weeks, on site at each member site, collect audit data and monitor audit tracker, support staff with surveys and any other audit week tasks as needed.

Undertake as required exploration of research publications/guidance/policies/journals to support with assuring that PCN priorities and strategies are aligned appropriately.

Organise and support any project evaluation, benefits realisation, and gap analysis work.

Lead on specific workstreams or small projects as required, reporting into a Project/Programme Manager.

Maintain and report on project budgets by working closely with PCN Digital & PCN Transformation Manager.

Meetings and events

Organising and planning: promote, schedule, attend, and minute project meetings, other related meetings/workshops/events.

Create a range of promotional and supporting information packs for events.

Support, participate and contribute to departmental events.

Co-ordinate provider visits where necessary.

Information and Finance

Support and inform the requirement and targeting of resources, monitoring, implementing, and evaluating jobs and delivery of financial recovery/savings plans by providing high quality information and analysis.

Contribute to the financial delivery of the agreed portfolio ensuring it is delivered on time.

Monitoring and checking project data, highlighting issues when necessary and presenting in an agreed manner.

Gathering, extraction and analysis of data from clinical systems, producing reports.

Producing/contributing to performance reporting for projects, ensuring team members or project members contribute as required.

Actively support and contribute to the development of key performance indicators for the successful assessment of performance of programmes and projects.

Develop and maintain databases required by job.

Maintain administrative and information resources.

General Administration

Assisting with arranging meetings.

Undertaking project/programme administrative duties as required.

Acting as a point of contact for projects/programmes.

Ensure project/programme filing is maintained in the agreed format ensuring that information is kept securely and is accessible to other members of the team.

To provide administrative support including when appropriate to all team members to support a range of department initiatives.
