Job summary
Social prescribing link workers connect people with local community activities and services that can help improve their health and wellbeing. A vital part of your role will be to build relationships with the people you are helping by listening carefully to what's important to them and what motivates them.
Working predominantly with Children and Young People and their families, you will spend time finding out 'What Matters to Them' and helping them to connect with support and services in the community to allow them to improve their general health and wellbeing.
Social Prescribing forms part of the NHS Long Term plan and focuses on early intervention and prevention and Personalised Care.
Jurassic Coast PCN works on behalf of Ammonite Health Partnership, Lyme Bay Medical and Barton House Medical Practice and this role will require a regular presence in Bridport, Beaminster and Lyme Regis.
Main duties of the job
- To work alongside clinical and non-clinical colleagues and partners to identify and receive referrals for individuals who would benefit from support
- To support individuals to access services and activities within their community to address their needs and desires
- To conduct 'What Matters to Me' conversations to allow people to identify their priorities and to build a relationship of trust
- To keep accurate records using agreed systems
- To manage a caseload and workload based on priority and the needs of the service
