Main duties of the job

This is a key role within Whitfield PCNs innovative hub model, where pharmacists and technicians work collaboratively across practices to deliver high-quality, patient-centred care. As a Senior Prescribing Pharmacist, youll independently manage long-term conditions, conduct structured medication reviews, and optimise prescribing to improve outcomes and reduce GP workload.

Working as part of a skilled and supportive pharmacy team, youll lead on clinical audits, medicines safety, and service development. You'll also contribute to PCN-wide strategies, mentor junior pharmacy staff, and play a vital role in reducing health inequalities through proactive population health management.

Hub working enables you to work flexibly across the network, sharing best practice, building strong relationships with GPs and wider ARRS roles, and supporting a consistent, high-quality pharmacy offer for all patients.

Whitfield PCN is seeking a proactive, clinically experienced Senior Independent Prescribing Pharmacist to join our multidisciplinary team. This is an exciting, full-time role within a forward-thinking PCN that values clinical leadership, hub working, and patient-centred care.

You will work across four GP practices, serving a population of approximately 40,000, contributing to high-quality care through structured medication reviews, vaccination clinics, quality improvement, and professional mentorship.

Key Responsibilities

1. Clinical Services

  • Deliver Structured Medication Reviews (SMRs) for complex patients (polypharmacy, frailty, LTCs).

  • Conduct general medication reviews for safety, efficacy, and optimisation.

  • Lead and participate in COVID-19 and seasonal flu vaccination clinics.

  • Support chronic disease management in line with current guidelines.

  • Prescribe and deprescribe within clinical competence, ensuring patient safety.

2. Medicines Management and Governance

  • Respond to and implement actions based on MHRA alerts and drug safety updates.

  • Ensure CQC compliance in medication-related clinical areas.

  • Undertake prescribing audits and support project-based improvements across practices.

  • Keep practices informed of stock shortages, medication changes, and relevant guidance.

3. Clinical Correspondence

  • Process hospital discharge summaries and outpatient letters relating to medicines.

  • Ensure timely and accurate medication reconciliation across transitions of care.

4. Education and Leadership

  • Educate practice teams on pharmacy services such as Pharmacy First and relevant PGDs.

  • Lead training sessions to raise awareness of safe and effective prescribing.

  • Mentor and clinically support junior pharmacists and pharmacy technicians on the CPPE Primary Care Pharmacy Education Pathway.

5. Prescribing Systems and Procedures

  • Support the implementation and monitoring of repeat prescribing policies and procedures across the PCN.

  • Assist with optimising the Electronic Prescription Service (EPS) to improve workflow and patient access.

6. Research and Innovation

  • Help establish Whitfield PCN as a research site, supporting engagement in primary care research studies and evidence-based practice initiatives.

  • Contribute to population health projects, health inequality reduction, and new models of care.

Key Attributes and Requirements

Essential:

  • Registered Pharmacist with the General Pharmaceutical Council (GPhC).

  • Qualified Independent Prescriber (annotated on GPhC register).

  • Primary care experience is essential, including familiarity with general practice systems.

  • Strong communication, clinical leadership, and problem-solving skills.

  • Ability to work independently and as part of a collaborative hub-based model.

Desirable:

  • Experience with EMIS, Ardens, or equivalent clinical systems.
