Job summary

This is an exciting opportunity for a Care Coordinator to join a newly established Care Home and Acute Visiting Service. The Care Coordinator role is essential to support the daily operations of both services. The post holder will work closely with our GP practice teams and PCN clinicians who are delivering care to our patients. The Care Coordinator will be required to manage acute visit requests from housebound patients, whilst working closely with our care homes supporting administration of clinical ward rounds and requests for ad hoc visits.

Main duties of the job

Alongside the acute visit management, the Care Coordinator will play a key role in ensuring proactive care is delivered efficiently by arranging visits for patients who are primarily housebound or living in care homes, this will include the management of long-term condition reviews/annual reviews. The Care Coordinator will be responsible for managing clinicians appointment ledgers and ensuring they accurately reflect the visits completed and will be required to manage allocations within the PCN and produce data reports outlining utilisation.

This role may be required to work at any of our PCN member practice sites.

Job responsibilities

Primary Duties

Responsible for the day-to-day running of the Home Visiting Service and Care Home Service, including the prioritisation of daily visits and updating the visit list based on capacity and liaising with clinical visiting staff to prioritise on clinical need.

Manage and prioritise workload to ensure patient and service needs are met

Manage clinical systems and appointment ledgers for both the Acute Visiting Service and Care Home Service

Manage team rotas and service capacity

Coordinate and manage administrative functions of both the Acute Visiting Service and Care Home Service

Build relationships with PCN Member Practices Clinical and Administrative teams as well as Care Home team members

Track and report service utilisation outlining allocations and booking completion across the Home Visiting Service

Communicate effectively with patients and their families/carers

Communicate with other members of the integrated care system if a patient needs to access other services

Coordinate patients long term condition annual reviews

Support administration of Personalised Care Support Plans (PCSP) for care home residents

Attend service meetings and contribute to service delivery and growth

Link with GPs and other clinicians in the PCN following visits to feedback patient outcomes where required

Refer complex cases to the multidisciplinary team meeting and complete administration after the meeting and communicate this back to practices.

Receive and collate information from hospital admissions and discharges and present this information to the team

Work with the PCN management team on other projects from time to time, where capacity allows, such as collating data for quality targets across the PCN or health inequality projects
