Job summary
We are looking for an Advanced Care Practitioner (Advanced Nurse Practitioner / Paramedic) to join our newly established Acute Visiting team who will be part of our Acute Visiting Service (AVS). The practitioner will provide both on the day acute visits for housebound patients within the PCN, as well as delivering proactive visits to support the ongoing heath needs of our population.
The post holder will be required to provide on the day visits for housebound patients who are acutely unwell and need a clinical assessment. The clinician will be responsible for prioritising visits according to clinical need and will be required to take a comprehensive history and may be required to liaise with secondary care and community colleagues to support coordination of patient care.
The post holder will work with PCN member practices and the PCN leadership team to develop a new and exciting service, this will include reviewing and developing clinical policy, practice, and procedures to ensure the service is safe, efficient, and effective for our housebound and GP member practices.
Main duties of the job
The Advanced Care Practitioner will work as an autonomous practitioner responsible for the delivery of the PCN DES requirements and AVS contractual requirements whilst working across the PCN multidisciplinary team, delivering care within their scope of practice. They will be working alongside a visiting GP who will also be involved in developing the service. Relationships are essential to this role and the practitioner will be required to always build meaningful working relationships with GP practice and PCN team members, including social prescribers, health and wellbeing coaches and pharmacists.
The post holder will have access to a wide range of support from the PCN and Member Practices, this includes regular supervision with a GP.
The AVS practitioners will work closely with our Enhanced Health in Care Home Team and will support each other where required. The practitioners in the AVS team may provide cover for EHCH team members where required to allow for seamless service delivery. Our EHCH colleagues will be working closely with care home residents and will provide weekly ward rounds with all of our registered care homes.
