Job summary

We are looking for an Advanced Care Practitioner (Advanced Nurse Practitioner / Paramedic) to join our newly established visiting team, the practitioner will be responsible for the Enhanced Health in Care Homes PCN DES delivery and will work alongside the Acute Visiting Service Team with an opportunity to support with acute visits to housebound patients where required. The post holder will be accountable for several clinical areas which support our patients who live in care homes, this will include providing a weekly ward round for patients living in our aligned homes to support with acute and routine health and care needs. Residents will be prioritised for review according to need, as assessed by the ACP and in conjunction with the care home staff. Alongside this the ACP will develop and refresh personalised care and support plans with residents annually. The post holder will be required to review patients following discharge from hospital and complete falls assessments after falls.

The post holder will work with local care homes and the PCN leadership team to develop a new and exciting service, this will include reviewing and developing clinical policy, practice, and procedures to ensure the service is safe, efficient, and effective for our care home residents and GP member practices.

Main duties of the job

The Advanced Care Practitioner will work as an autonomous practitioner responsible for the delivery of the Primary Care Network (PCN) Enhanced Service requirements. Relationships are essential to this role and the practitioner will be required to always build meaningful working relationships with GP practices and PCN team members as well as care home staff.

This role opens up an exciting opportunity to build relationships with care home managers, reduce unnecessary prescribing, and to be an advocate for what matters to residents and their families. The post aims to reduce GP workload whilst supporting GPs to provide appropriate care and prevent over medicalisation.

The post holder will have access to a wide range of support from the PCN and Member Practices, this includes regular supervision with a GP.

Primary Duties

Uses expert knowledge and clinical skills to deliver holistic care to care home residents registered with Southampton West PCN

Provides safe, evidence-based, cost effective, individualised patient care

Assesses diagnoses and treats patients attending primary health care services with a range of acute, non-acute, and chronic medical conditions

Assesses diagnoses and treats patients in care home who require acute medical attention and refer to appropriate agencies

Directly admits patients to secondary care hospital in acute medical need

Refers patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations

Interprets a range of diagnostic tests

Works within professional guidelines and codes of conduct

Works within all relevant policies and procedural guidelines e.g. infection control, chaperoning, risk management

Communicates information to patients and carers sensitively and with due regard to confidentiality

Has a good understanding of patient capacity and best interests decisions

Contributes towards targets under the PCN Enhanced service and Quality and Outcomes Framework

Undertakes and completes quality improvement projects

Undertakes annual reviews and care plans for patients including medication reviews with support from PCN pharmacists

Work with PCN ARRS roles including Clinical Pharmacists and Social Prescribers

Accurately records clinical consultations on practice clinical systems ensuring the correct use of SNOMED codes and clinical templates

Understands and implements safeguarding protocols and understands how and to whom to refer and escalate when required

To prescribe safe, effective and appropriate medication

Develops meaningful positive working relationships with local frailty teams and multidisciplinary teams
