Job summary

The post holder will work as part of the Health Centre Team in collaboration with the general practitioners and develop an effective, quality driven practice nurse service. The post holder will promote integrated nursing practice within the wider nursing team underpinned by the development of evidence-based practice guidelines, for shared delivery of care to the whole practice population. Further, to ensure clinical effectiveness by monitoring of key quality markers relating to guidelines

Main duties of the job

Practice independently in accordance with NMC code of conduct and with reference to practice guidelines, appropriately referring to GPs.

To receive and assess emergency patients and any incoming medical queries either by phone or face to face appointment and either treat or refer as appropriate.

Management of chronic diseases, in particular disease areas of Asthma and COPD and dressing suture removal, ear syringing, management of minor illnesses, vaccinations, immunisations, treatment of warts and verrucae either with or without liquid nitrogen etc following relevant training and in the absence of nursing colleagues. any area as per procedure with training guidance.

To encourage health promotion and life-style counselling of patients as appropriate.

Ensure continuation of own personal and professional development facilitated by utilisation of clinical supervision networks.

Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary.

Administration & Professional Responsibilities

Ensure accurate note of all consultations and treatments are recorded in the patients computer records, as required.

Ensure accurate completion of all necessary documentation associated with patient health care within the practice.

To be involved in National Service Frameworks and Clinical Governance as required.

To participate in the production of new and revised protocols for the practice in conjunction with the GPs and Practice Manager.

Attend and participate in practice meetings, including the Multi-Disciplinary Health Care Team Meetings as required.

To maintain up-to-date personal professional development and awareness through reading and attendance on courses as agreed with the lead GP.

To maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the practice with appropriate regard to confidentiality.

To ensure that the key requirements of the current Health and Safety at Work legislation are understood, in that it is the personal responsibility of every employee to work and behave in the manner which protects the safety of their colleagues and the patients and to ensure their work place is maintained in a safe, clean and tidy condition at all times.

The post holder will contribute to audit as requested by the Partners of the Health Care Centre

Travel vaccinations

Routine immunisations for infant, child and adult

Cervical smears

Family Planning advice

Diabetes, Asthma and COPD management

Support practice to achieve QOF.

Requesting appropriate pathology tests to support patient diagnosis.

Act as a conduit to the patients registered GP for further advice where appropriate.

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.

Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary.

Advise patients on general health care with referral to GPs as necessary.

Communication and relationships

To liaise and communicate with staff, managers, patients, and service users at all levels

Develop effective channels and systems of communication and ensure colleagues and staff are kept informed in a timely and appropriate manner

To ensure that confidentiality is always maintained in conjunction with the Practice's Confidentiality Policy

Play an active role as members of interdisciplinary teams

Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a person and their family

  • RGN - First Level Nurse/Adult Branch Specific Diploma or Degree

Desirable

  • Diploma or equivalent in Chronic disease Management (or working
  • Towards)
  • ENB 998 or equivalent

Experience

  • 3 years post registration experience
  • Theoretical/practical knowledge of chronic disease management programmes and NSFs and willingness to work towards appropriate qualifications
  • Demonstrate the ability to give a high level of care to patients
