Are you an enthusiastic,
experienced paramedic with an interest in frailty, looking to expand your
skills further? A new opportunity has
arisen for a compassionate Community Paramedic to join the Mewstone PCN Older
Person team. You will work as part of a multidisciplinary team responsible for
providing a comprehensive frailty service, supporting patients requiring care
in various settings whilst providing appropriate information, advice, or
referrals as per agreed protocols.
Our objectives are to prevent
unnecessary hospital admissions, to improve care coordination, increase the use
of anticipatory care plans, a reduction in falls and improved management of
chronic conditions.
The role will work with our
Older Person team across care homes and housebound patients, you will work
collaboratively with the team to ensure the delivery of safe and effective care
for patients, including advanced care planning, chronic disease management,
treatment, preventative care, screening, and education. Ensuring the safety and
wellbeing of service users and always working in a manner that promotes dignity
and improved patient experience through the adoption of person-centred care
principles.
As part of the role the Community
Paramedic will play a key role in supporting local practices with their acute
visits for older people and link into the Ageing Well MDTs, this will be done
in conjunction with the Care Co-ordinator team. The role will require a
collaborative approach with place based teams.
Main duties of the job
We are looking for an enthusiastic individual that
have advanced skills in patient care delivery. These skills will include holistic,
evidence-based patient care, illness assessment and diagnosis
The post holder will have special interest in
primary care and acute assessment of patients within care homes, their own
homes, liaison with colleagues from the multidisciplinary team and GP practices
to support the patient at home.
We are looking for individuals who wish to make a
difference to the care and treatment of patients and can embrace the 5 year
forward view and NHS Long term plan to ensure the changes are delivered within
both primary and urgent care.
About us
Mewstone Primary Care Network has a list size
of approximately 32,000 patients. The PCN practices are Church View Surgery,
Wembury Surgery, Yealm Medical Practice and Dean Cross Surgery, the practices
share the same ethos of supportive learning.
You will work alongside our GPs, nurse teams,
administrators and extended workforce including clinical pharmacists, pharmacy
technicians, first contact physiotherapist, mental health practitioner, social
prescribing link workers, health & wellbeing coaches and care
co-ordinators.
All network practices are high achieving

team that completes care home and home visits for primary care practices across
the PCN. Providing on the day assessment for patients to prevent hospital
admission. The role is to provide care for patients with a variety of
illnesses, completing clinical assessment and formulating diagnosis which is
discussed with the GP to formulate a treatment plan. The post holder will work
collaboratively within the team and complete referrals to relevant
organisations to support patient care.
The post holder will support the delivery of
high-quality care and will support service development and improvement of frailty
care, working within the team, across practices and with partners where
appropriate. Committed to the development of innovative practice and expanding
evidence-based care which will contribute to the enhancement of the paramedic
role. The post holder will:
- Assess patients health care needs using multiple
methods and make professional autonomous decisions based on level of
clinical knowledge and skills while remaining within your scope of practice
- Make differential diagnosis using decision making
and problem-solving skills.
- Triage and prioritise patients appropriately so
that they get the care need.
- Screen patients for disease risk factors and early
signs of illness.
- Order and interpret necessary investigations,
discuss results and plans with a doctor and implement those plans.
- Provide treatment individually or as part of a
holistic care team by using relevant referral pathways for example two week
waits, imaging, etc.
- Provide health education and advice to patients and
support them with their decisions.
- Work collaboratively with other health care
professionals, carers, family and support structures.
- Undertake the collection of pathological specimens
including blood samples, swabs, etc.
- Ensure that accurate documentation of all
consultations and treatments are completed in the correct manor and all other
administrative duties are completed.

sessions and peer support groups.
- Complete home visits, face to face appointments in
clinic and discuss treatment plans via telephone, depending what is needed for
the patient and considering their needs.
