We are seeking a highly skilled and autonomous Advanced Nurse Practitioner to join our dynamic and progressive team. The ANP will deliver high-quality, evidence-based clinical care to patients across a wide range of acute, chronic, and complex conditions. Working within a multi-disciplinary team, the ANP will be responsible for assessing, diagnosing, treating, and managing patients within the scope of their advanced practice.

The successful candidate will be an independent prescriber and able to work both independently and collaboratively to meet the healthcare needs of our diverse population. This role is essential to supporting continuity of care, improving access, and enhancing patient experience across our network of practices.

• The successful candidate will be a registered nurse with current NMC registration and will hold a Masters level qualification in Advanced Clinical Practice (or be working towards this), along with an independent prescribing qualification (V300)

Experience

• Professional Registration
• Current registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse.
• Masters degree in Advanced Clinical Practice (or equivalent postgraduate qualification in an advanced practice pathway).
• Independent and Supplementary Prescribing qualification (V300).
• Significant post-registration clinical experience in a relevant setting (e.g., primary care, urgent care, community services).
• Demonstrated ability to assess, diagnose, treat and manage patients with a range of acute, chronic, and complex conditions.
• Advanced clinical assessment and decision-making skills.
• Ability to work autonomously within scope of practice.
• Strong understanding of evidence-based practice, clinical governance, and current NHS policies and guidelines.
• Confident in prescribing safely and appropriately in line with national standards.
• Competent in ordering, interpreting and acting on diagnostic tests (e.g. bloods, ECGs, X-rays).
• Excellent communication and interpersonal skills across all levels of staff and patient populations.
• Ability to work effectively as part of a multidisciplinary team and contribute to collaborative care.
• IT and Record-Keeping
• Competent in the use of clinical systems (e.g. EMIS Web, Docman) and maintaining accurate, timely clinical records.
• High level of professionalism, integrity, and accountability.
• Strong commitment to patient-centred care, clinical excellence, and continuous professional development.
• Understanding of CQC requirements, clinical risk assessment, and regulatory frameworks relevant to primary care.

Desirable

• Non-medical prescribing mentorship experience.
• Previous experience working within general practice or Primary Care Networks (PCNs).
• Experience in urgent care, home visits, or out-of-hours services.
• Exposure to and confidence in managing mental health presentations
• Involvement in clinical audits, research, or quality improvement initiatives.
• Experience in supervising, mentoring, or developing junior staff or students.
• Ability to support service redesign or development in response to changing patient needs.
• Awareness of digital health tools or remote consultation platforms (e.g., AccuRx, AskFirst).
• Familiarity with local pathways, referral systems, and working with multi-agency teams (e.g., social care, mental health services).
• Understanding of population health, health inequalities, and personalised care models.
• Flexible and able to adapt to the evolving demands of primary care transformation.
• Experience in managing long-term conditions such as diabetes, asthma, COPD, and hypertension.
