Job summary

The post holder's primary responsibilities are to be a team player within the Primary Care Knowsley (PCK) Call and Recall Team (CRT), taking ownership of a particular practice list or specific group of patients as part of a wider call and recall system. The post holder will support patient care and Practice objectives by calling patients to attend for reviews with the clinical team. Making efficient use of available clinical time by proactively engaging with patients using a variety of channels of communication (e.g. letter, email, SMS text messaging and phone), the post holder will offer a positive and friendly image when communicating to patients.

Job description

Job responsibilities

The duties and responsibilities to be undertaken by the CRT administrator may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the CRT Manager, dependent on the current and evolving practice workload and staffing levels:

Build on existing systems to provide an effective and efficient call recall system to patients including but not limited to:

- Chronic disease management such as diabetes, asthma, heart disease etc

- Screening programmes support the follow up of patients called through national programmes e.g. AAA and cancer screening: cervical, breast, and bowel.

- Immunisations and Vaccinations (child immunisations, influenza, COVID-19)

- Health Promotion including NHS Health Checks and Diabetes Prevention Programme

Monitor performance against Practice standards or frameworks such as the Quality and Outcomes Framework.

Identifying patients who through disability or illness require alternative means of communication (e.g. large font, or phone calls rather than standard letters).

Identify carers and offer them information on the support available through the Practice.

Notify the patients GP if there are concerns regarding vulnerable adults or children

Participate in CRT and Practice meetings as required.

Work with the management team and clinical team to ensure efficient use of clinical time through active call management of patients and reducing DNAs.

Monitor and report on key performance indicators for the call and recall systems, working with the CRT Manager to develop clinical system searches and reports and other information systems as necessary.

Receive and make telephone calls as required. Take messages, ensuring accuracy of transcription and pass them onto the appropriate person promptly.

Enter patient information on to the clinical system (EMIS Web) as required.

Undertake any other additional duties appropriate to the post as requested by the CRT Manager or other senior members of the team.

General Duties

Be involved in the maintaining and encouraging of adequate security measures e.g. locking doors, setting security alarm where relevant.

Leave desk tidy at the end of each day.

Maintain the Health and Safety of all staff, patients and visitors.

Comply and familiarise yourself with the Practices policies and procedures

Carry out any other duties as reasonably requested by the CRT Manager.
