Job summary

Main duties of the job

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within or outside of the organisation.

Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

Screening patients for disease risk factors and early signs of illness.

In consultation with patients and in line with current practice disease management protocols, developing care plans for health management, offering support and health education.

Making appropriate and timely referral of patients for further opinion and treatment

Recording clear and contemporaneous consultation notes to agreed standards including the recording of appropriate information for the Quality and Outcomes Framework and Enhanced Services.

Appropriate medicine prescribing for both acute and repeat requirement in accordance with Pan Mersey prescribing formulary and guidelines whenever this is clinically appropriate.

Timely assessment of incoming electronic and paper correspondence. Using the appropriate electronic systems.

Participate in training plans of GP registrar, medical and nursing students as required

KEY RESPONSIBILITIES

Clinical Care

  • Act as Advanced Nurse Practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
  • Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat or refer patients
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-
  • based treatment methods into a management plan
  • Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.
  • Work with patients and their carers in order to support compliance with and adherence to prescribed treatments
  • Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members
