Job summary

NEMS offers an out of hospital pathway for agreed medical conditions. GPs can refer to the NEMS pathways team through a dedicated line so that our clinicians can review the urgency and clinical nature of the patients health issue.

Many of the patients referred onto NEMS Pathways are seen by NEMS clinicians based at our facilities at Queens Medical Centre and PC24. Here, they can facilitate a further assessment, in 95% of cases referred a safe alternative to a hospital admission is achieved.

Main duties of the job

  • To provide strong clinical presence and role-model for excellent clinical practice, to support the pathway team to carry out assessments of patients attending within a determined clinical pathway such as Deep vein Thrombosis Pathway, Renal Colic, Urgent bloods and Cellulitis pathways and more.
  • To provide leadership in supporting growth and administration of treatment in line with policies, procedures, and guidelines, while developing relationships with associated partners or agencies.
  • Ensuring annual appraisals, personal development plan and performance management are adhered to.
  • Work in collaboration with the Clinical director and Heads of Clinical Service to comply with operational performance targets, ensuring effective use of resources to assure provisions of a safe and efficient service.
  • To ensure patients have a satisfactory experience by evaluating patient processes and supporting redesign of care pathways through audits, incident review and patient feedback.
  • To inspire, motivate and empower all members of the multiple disciplinary teams.
  • This post holder will have a strong clinical and operational background in primary and/or urgent Care. The role will consist of an average 60% clinical and 40% strategic leadership.

  • Liaise and communicate clinical information verbally and via technical links with NEMS and health economy partners to ensure continuity of care for the caller.
  • Maintain accurate, contemporaneous record keeping of all interventions with patients, fully utilising computer software systems in place and/or other mediums.
  • Maintain confidentiality at all times in accordance with legislative and policy requirements.
  • Act as the patients advocate when required to.
  • Participate and assist in service evaluation, clinical audit and research as required.
  • Liaise directly with GPs and Senior Primary Care Practitioners regarding medicines outside of the individuals normal sphere of competence and medicine-related uncertainties.
  • Participate in the investigation of complaints, incidents and accidents.
  • Undertake safe clinical practice within health and safety guidelines.
  • Work within and be aware of the organisations clinical policies and guidelines.
  • Maintain own awareness of current health policy relevant to practice.
