Job summary

NEMS have an exciting opportunity for an experienced Nurse to join the team as a Pathways Primary Care Practitioner! Best of all you won't work bank holidays or weekends!

Main duties of the job

NEMS Pathways is currently based within NEMS at QMC, adjacent to the Queens Medical Centre Emergency Department (ED). We provide a range of primary care alternatives to ED attendance or hospital admission for patients with urgent health and care needs.

Working under the supervision and direction of the Pathways Lead Nurse and other experienced Primary Care Practitioners, you will carry out assessments of patients attending NEMS within determined clinical pathways.

The service operates between 10am and 10pm Monday to Friday and is staffed by autonomous primary care clinicians and practitioners supported by a Pathways Coordinator/Associate Practitioner and skilled healthcare assistants.

The shift patterns are 09:30-16:00 or 15:30-22:00.

Clinical Responsibilities:

  • Always practice within scope of competence, recognising limitations of clinical practice.
  • Undertake clinical assessments of patients healthcare needs and provide clinical advice in accordance with relevant policies and guidance, administer treatments either prescribed or under PGD and facilitate onward referral to other professionals if required.
  • Liaise and communicate clinical information verbally and via technical links with NEMS and health economy partners to ensure continuity of care for the caller.
  • Maintain accurate, contemporaneous record keeping of all interventions with patients, fully utilising computer software systems in place and/or other mediums.
  • Maintain confidentiality at all times in accordance with legislative and policy requirements.
  • Act as the patients advocate when required to.
  • Participate and assist in service evaluation, clinical audit and research as required.
  • Liaise directly with GPs and Senior Primary Care Practitioners regarding medicines outside of the individuals normal sphere of competence and medicine-related uncertainties.
  • Participate in the investigation of complaints, incidents and accidents.
  • Undertake safe clinical practice within health and safety guidelines.
  • Work within and be aware of the organisations clinical policies and guidelines.
  • Maintain own awareness of current health policy relevant to practice.
  • Be aware of the impact your decisions may have upon the patient and their General Practitioner.
  • Liaise with other professionals to facilitate direct referrals to other health care providers within the policies of the organisation.
  • Participate in the mentorship of health professionals and students on recognised training programmes, e.g. student nurses, ECPs Practice Nurses etc.
  • Recognise and act upon opportunities through consultation to promote public health.
