JOB DESCRIPTION
Diabetes Care Coordinator - Band 4
Responsible to; PCN Business Manager
Accountable to; PCN Clinical director / Bexhill PCN
Contract length: Fixed term for 18 months with the view to make a permanent contract after March 2024
1. Job Summary
The Diabetes care coordinator
role will work as a key member of the Bexhill PCN team in supporting the
delivery of the care processes for Diabetes prevention and All forms of
diabetes affecting the people living in Bexhill and registered with Bexhill GP
Practices.
The Diabetes care
coordinator will work closely with healthcare professionals, patients and local
communities to encourage patients at high risk of type 2 diabetes to access
lifestyle interventions to reduce their risk of developing the condition and will
also take an active role in quality improvement activities and partnership
working. They will proactively support patients with diabetes to engage with
the care processes offered by practices. They will support practices to improve
diabetes care among the vulnerable people, the regular non-attenders,

2.
Main
responsibilities
Includes but not restricted to:
Providing comprehensive support to the delivery
of the NDPP across Bexhill PCN - including supporting GP practices to identify
and refer eligible patients and to encourage patients at high risk of type 2
diabetes to access lifestyle interventions to reduce their risk of the condition
Promoting diabetes prevention and awareness
Promoting self-management of diabetes,
encouraging uptake of all care processes, signpost people to appropriate
services to help them with their Physical/ Psychological/social health.
Help PCN and practices to tackle health
inequalities
Work closely with the ICDC part take in MDT
meetings and coordinate care of people with diabetes.
Patient engagement including engaging with BAME
populations and deprived communities
Being the key link between the people with
Diabetes, carers, GP practices and PCN.
Undertaking information governance training and
to ensure confidentiality agreements are signed prior to working directly with
patient data within practices.
Arranging, attending, and contributing to relevant
meetings with actions followed up as required
Undertaking administrative duties such as
photocopying and mail-out distributions.
Ensuring all urgent and/or confidential
communications are received and distributed from/to relevant parties in a
timely manner.
Arranging meetings with staff, both clinical and
non-clinical
Supporting engagement, planning, reporting and
administrative needs
Gathering information and undertaking enquiries
as and when necessary
Supporting other projects as required
Other related activities:
Completing NHS mandatory training
Identify personal education needs and skills
development with the registered practitioner
Actively engage with relevant partners including
Diabetes UK to utilise their expertise and experience and support
Demonstrate an awareness of the limits of own practice
and knowledge and when to seek appropriate support/advice
To be aware of Health and Safety policies and
procedures with regards to Visual Display Units and seating
To listen with empathy to patients and carers, ensuring
discretion and confidentiality

and maintain health, safety and security of self and others in own work area
c) Ensure
own knowledge of appropriate professional standard and Trust professional
guidelines and that these are applied to practice

to other practitioners when health outcomes or risks and needs are beyond own competence
and scope of practice
