Job summary

DHC is offering a great opportunity to be part of providing really high quality and innovative care to frail patients in the Dorking area.

As a Frailty Care Coordinator, the successful applicant will work closely with clinical teams helping to provide optimal care for frail patients. The role involves some patient contact by telephone and in person, and a positive, respectful, and compassionate nature is necessary when delivering a high-quality, safe and effective service with the highest standards of confidentiality.

The Care Coordinator will be provided with all necessary training to fulfil the project to its full potential.

Main duties of the job

DHC is looking for a motivated and compassionate person who either has, or is working towards, a healthcare certificate, and who enjoys patient contact and the challenge of providing great quality care. The role involves some home visits and some clinical care, and you will work with a variety of clinical and non clinical staff to ensure that frail patients receive the most joined up and appropriate care possible.

You will use data searches to seek out patients for care, and support with innovating pathways for frail patients, along with a team of healthcare professionals. You will also liaise with care homes in the area to optimize the relationship with primary care for patient benefit.

The PCNs are critical to the changing NHS and are the focal point of all locally developed planning and service delivery.

  • Proven experience of managing a complex administrative role
