Job summary

DHC is recruiting a Psychological and Wellbeing Care Coordinator to join our Live Well team. This exciting role is a fantastic opportunity for anyone looking to develop their career within primary care mental health services.

This is a service delivery assistant role, working within the multidisciplinary Live Well Team, to support the clients journey from referral to discharge and assist clinicians with interventions. The Care Coordinator will work closely with clinicians to develop care plans and co-ordinate and monitor a range of patient services.

It is essential that the person has a positive and compassionate nature and can provide exceptional customer service. This role requires sensitivity and an understanding that patients with any medical needs, complex or general, should be treated with respect.

The Care Coordinator will deliver a high-quality service for patients in a safe and effective working environment, whilst maintaining the highest standards of confidentiality. They will be a motivated person with good initiative and organisational skills and have excellent interpersonal and team working skills.

Main duties of the job

The ideal candidate will need proven administrative and IT skills and have experience of managing a complex and varied workload. You will also ideally have had experience of working in a similar role within healthcare or in the public sector, leisure or voluntary sectors, working with the general public.

In addition, you will need to relish the challenge of working under pressure, as part of a thriving and growing team both within a GP practice and as part of the wider Dorking Healthcare organisation.

Travel between sites may be necessary so whilst not essential, a car driver would be ideal.

DHC is a GP Federation, representing 140,000 patients providing a wide range of quality healthcare services including elective care, Talking Therapies, community healthcare and primary care across Surrey. We are registered with the CQC (Care Quality Commission) and we are an accredited AQP (Any Qualified Provider).

GP practices have been brought together into Primary Care Networks (or PCNs) to offer services in the local community. There are four Primary Care Networks as part of the DHC Federation, Redhill Phoenix PCN, Dorking PCN, South Tandridge PCN and Integrated Care Partnership PCN. The PCNs are critical to the changing NHS and are the focal point of all locally developed planning and service delivery.
