Job summary
An
exciting opportunity for an experienced Clinical Pharmacist has arisen within
our team. We are looking for a highly experienced Clinical Pharmacist to join
our friendly team, located in the picturesque village of Godstone, Surrey.
We
demonstrate strong teamwork throughout the practice and pride ourselves on
having a happy workforce. The successful candidate would benefit from being
part of a much wider team of Clinical Pharmacists who sit under the four
Primary Care Networks who sit under Dorking Healthcare.
Main duties of the job
For the new Structured Medications Review
and Optimisation requirements this will include tackling over-medication of
patients, including inappropriate use of antibiotics, withdrawing medicines no
longer needed through NHS-led programmes such as low priority prescribing and
medicines optimisation more widely. For Enhanced Health in Care Homes residents
will benefit from regular clinical pharmacy-led medicines reviews.

The contract for Pond Tail Surgery was taken over by DHC in January 2022 and has gone from strength to strength under their guidance. The team is growing rapidly and we now have a full complement of permanent staff including x3 clinical lead GPs. We are a vibrant and friendly team who are devoted to delivering a high standard of healthcare to our patients.
DHC is a GP Federation, representing 140,000 patients providing a wide range of quality healthcare services including elective care, Talking Therapies, community healthcare and primary care across Surrey. We are registered with the CQC and we are an accredited AQP.
DHC is working at the centre of the local health system to evolve and deliver integrated care and that requires strong partnerships with many organisations. In all of our services, we aim to put the patient at the centre of their care, make sure our services are easy to access by local patients and reduce waiting times for patients.
GP practices have been brought together into Primary Care Networks (or PCNs) to offer services in the local community. There are four PCNs who are part of the DHC Federation, Redhill Phoenix PCN, Dorking PCN, South Tandridge PCN and Integrated Care Partnership PCN. The PCNs are critical to the changing NHS and are the focal point of all locally developed planning and service delivery.

- In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
