Job summary
This role supports patients across the geography of three Primary Care Networks to manage their cardiovascular condition at home.
Working with teams of GPs, administrators, managers, and pharmacists the successful candidate will coordinate the activities of the @home services (blood pressure and heart failure) ensuring relevant observations are monitored, assessed, reported, and acted upon.
Talking with patients to support their use of services forms part of the job responsibilities along with the administration of service activity and accurate recording of patient details and cohort lists. Contacting and referring to other NHS services adds to the variety of tasks necessary to deliver the service supported by the role.
Contributing to the way the services work, whilst ensuring impact to regular Practice activities is minimized means using clinical capacity efficiently; this is a key element of the role which will require good communication between individual colleagues and multidisciplinary teams. The role is dynamic and can be fast paced to meet these requirements delivering high quality care.
The aim of the role is to optimize patient outcomes by using Practice resources efficiently, the successful candidate will be helping improve the effectiveness of health care provision In the homes of patients.
Main duties of the job
- Identifying patient cohorts,
- managing the submission of patient observations,
- using inhouse assessment methods for observations,
- meeting with pharmacy to review patient records,
- actioning outcomes,
- detailed and accurate information recording,
- communicating advice / outcomes to patients
- reporting service activity,
- planning and delivering phased service roll out,
- liaising with Practice colleagues.
