Care Coordinator - Leith Hill Practice, Capel

Job summary

We are looking for a dynamic, hardworking, and motivated individual to join the Dorking PCN team of care coordinators. The post holder will be based at Leith Hill Surgery in Capel and work as a team with the coordinators in our other practices in Dorking and across the Federation.

In this exciting position you will really make a difference for our patients and will take a key role in working with the clinicians in your practice and wider primary / community teams of professionals to ensure patients receive interventions and support as appropriate to their condition. The role will include managing and maintaining a case load of patients with Long Term Conditions, monitoring patient information such as hospital admissions and supporting people by referring or sign posting to appropriate local services. There will be a strong administrative and data function to the role as well as coordination between the practice, patients and carers and other services.

This is a developmental role and the successful candidate will be fully supported in attending the accredited care coordinator education programme and training in supporting the individual needs of the people on your case load.

This role is a part-time job share working 20.5 hours over 3 days a week.

Main duties of the job

The ideal candidate will need proven administrative and IT skills and have experience of managing a complex and varied workload. You will also ideally have had experience of working in a similar role within healthcare or in the public sector, leisure or voluntary sectors, working with the general public.

In addition, you will need to relish the challenge of working under pressure, as part of a thriving and growing team both within a GP practice and as part of the wider Dorking Healthcare organisation.

Travel between sites may be necessary so whilst not essential, a car driver would be ideal.
