Job summary
- We are looking for an enthusiastic administrator who has excellent organisational, multidisciplinary and communication skills with experience working in the general practice setting.
- Provide coordination and navigation to patients across the organisation requiring medication support through processing medication requests, triage or signposting to the correct pharmacy service.
- Support patients with their understanding of the pharmacy services provided by the Medicines Management Team.
Main duties of the job
Answering Pharmacy and Surgery phone lines
Triage requests and queries from phone calls to the team.
Support with drug monitoring, drug safety
Manage care home inbox and coordinate workflow within the team
Manage prescribing hub email inbox including triage of work to appropriate team member.
Work alongside with pharmacy hub as required
Coordinating the receipt of shared care agreements or SIDC's from secondary care
Coordinating new patients EMIS online proxy forms and registrations for care homes.
Sending out hub communications to homes, pharmacies and surgeries (bank holidays, PLT, news blast for urgent matters).
Collation and management of shared documents on the appropriate platform ensuring the latest versions are available and are made available in a timely manner.
Creation of EMIS patient searches to support the coordination of project work across the medicines management team.
Liaising with pharmacies to ensure safe, cost effective ordering of medications to reduce waste.
Provide coordination and navigation to patients across the PCN requiring medication support through triage or signposting to the correct pharmacy service.
Support patients with their understanding of the pharmacy services provided by the Medicines Management Team.
Work closely with the Frailty and Medicines Management teams to assist with Nursing and Residential ward rounds.
Support patients to utilise decision aids and tools in preparation for a shared decision-making conversation.

We are a 55k patient practice working from 5 sites in Central Lewisham. TLCP is a Partnership and PCN in its own right with no other practices involved.

We are working with partner organisations at all level within Health and Social Care from other PCNs in the locality to national initiatives. You will be entitled to Annual Leave, Bank Holiday and Study Leave and supported by the clinical team/admin team within the practices.
We are seeking dynamic professional colleagues keen to join our clinical team. TLCP has 14 GP partners, 15 Salaried GPs , 21 in our nursing team and 8 in our pharmacy team
TLCP is a teaching practice for Medical and Nursing staff, it has a full complement of non-clinical managers and staff.
