Job summary

We are looking a newly qualified or experienced Physician Associate to join our multidisciplinary team. The opportunity has arisen as the partnership is looking to boost its numbers and diversify the disciplines of our front-line clinical team.

Main duties of the job

To be responsible for the provision of a clinical service, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.

The physician associate will be required to work autonomously in the practice, providing a range of services such as assessment, diagnosis, treatment, telephone triage, clinical decision-making and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure when required.

We are a 58k patient practice working from 5 sites in Central Lewisham. TLCP is a Partnership and PCN in its own right with no other practices involved.

We are working with partner organisations at all level within Health and Social Care from other PCNs in the locality to national initiatives. You will be entitled to Annual Leave, Bank Holiday and Study Leave and supported by the clinical team/admin team within the practices.

We are seeking dynamic professional colleagues keen to join our clinical team. TLCP has 21 GP partners, 15 Salaried GPs , 21 in our nursing team and 8 in our pharmacy team

TLCP is a teaching practice for Medical and Nursing staff, it has a full complement of non-clinical managers and staff

Job description

Job responsibilities

The following are the core responsibilities of the physician associate. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels. The physician associate will work within their scope of clinical practice and:

a. Identify, signpost or refer patients at risk of developing long-term conditions, preventing adverse effects on the patient’s health

b. Provide routine care to patients as required in accordance with clinical based evidence, NICE and the NSF

c. Examine, assess and diagnose patients and provide clinical care/ management as required

d. Triage patients and provide the necessary treatment during home visits

e. Triage and treat or refer patients who want to see a GP/ACP.

f. Collect pathology specimens as required

g. Maintain accurate clinical records in conjunction with extant legislation

h. Ensure clinical codes are used effectively

i. Ensure continuity of care, arranging follow-up consultations or reviews as necessary

j. Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes

k. Chaperone patients where necessary

l. Prioritise health issues and intervene appropriately

m. Recognise, assess and refer patients presenting with mental health needs

n. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)

o. Liaise with external services/agencies to ensure that the patient is supported appropriately (vulnerable patients etc.)

p. Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times

q. Support the clinical team with all safeguarding matters, in accordance with local and national policies

r. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

s. Deliver opportunistic health promotion where appropriate
