Job summary

London Fields PCN is seeking a Pharmacy Technician to join the pharmacy team and the wider multidisciplinary team. The post holder will support the Clinical Pharmacists to ensure the delivery of safe, effective and efficient systems for repeat prescribing, aiming to reduce medicines wastage, encouraging and implementing the principles of medicines optimisation and maximising patient outcomes. In addition, they will provide technical and administrative support to the Clinical Pharmacists and Clinicians as well as support general practice teams with medication-related queries from patients; including face-to-face and telephone support to patients and primary care staff.

Main duties of the job

We are looking for a collaborative, motivated pharmacy technician who is flexible to respond to the developing nature of the role and the needs and priorities of the service users. Under the direction of Clinical Pharmacists, we expect that you will be able to work independently and manage your own workload. You will need excellent communication skills to be able to liaise with patients, primary care staff, community pharmacists and other partners.

You will be working across the practices in the PCN and no two days will be the same, allowing for a varied and fulfilling role. You will become part of a PCN Pharmacy network team, with full support from both, the PCN and the North East London ICB. Training and development support through access to the CPPE Primary Care Pharmacy Education Pathway will be encouraged and supported.

About us

Groups of GP Practices in City and Hackney have come together to form 8 Primary Care Networks (PCNS) each covering a total population of between 30,000-56,000. The purpose of these PCNs is to enable GP practices to work together in a collaborative way to develop and deliver network-based services that respond to the needs of the local population.

Job responsibilities

Key duties and responsibilities

To support the PCN Clinical Pharmacists in the delivery of clinical pharmacy services;

To perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the GP or Clinical Pharmacist;

To be responsible for managing the system for high-risk drug monitoring across the PCN;

To process repeat medication requests, including the conversion of acute requests to repeat medicines and changing quantities of medicines;

To request issue of prescriptions in line with agreed clinical protocols and within your competency and directing queries to an appropriate clinician;

To liaise with clinicians and administrative staff to resolve medicine-related queries;

To provide advice and guidance to patients regarding medication optimisation and lifestyle e.g. inhaler techniques;

To provide basic medication review services to patients, under the supervision of a Clinical Pharmacist, via clinics in practices, domiciliary visits and in residential and nursing homes and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care;

To support the Clinical Pharmacists to deliver Structured Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR;

Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients;

To support practices in the achievement of prescribing Quality and Outcomes Framework targets, and other medication management targets;

To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken;

To monitor practice prescribing and adherence to practice, local and national prescribing policies;

To assist with the development and review of medicine audits;

To receive, document and implement actions for MHRA drug recalls, working under the supervision of the Clinical Pharmacist;

To undertake specific work/prescribing initiatives under the support of a PCN Clinical Pharmacist;

Conduct medication reconciliation following discharge from hospital, clinic visits and other transfers of care e.g., new patients, care home residents), to ensure that medication changes are safely implemented in a timely fashion, liaising with primary care and secondary care colleagues, where necessary;

To promote cost effective, safe, evidence-based prescribing in accordance with local formulary, medication optimisation strategy and national guidance;

To establish positive working relationships and effective communication with network practices, integrated care health and care teams, and a range of local voluntary sector and community organisations;

To contribute to the development of the service, e.g., developing service pathways, paperwork and sharing good practice across network practices;

To attend and participate in local and practice clinical meetings and other multi-disciplinary meetings where necessary.
