Job summary
The Outreach Psychotherapist is a
novel position being introduced within a multi-disciplinary team framework to
cater to the mental health and overall well-being of individuals experiencing
homelessness or at risk of it in Peterborough. Collaborating closely with a
Mental Health-specialised GP and a Service Lead who will oversee management and
administrative aspects, this role aims to offer psychotherapeutic interventions
to referred rough sleepers who face moderate mental health challenges alongside
intertwined health and social care needs, including substance misuse. The
interventions will occur in diverse settings like street outreach or client
accommodations. This initiative aligns with an integrated care model for Rough
Sleepers encompassing housing, substance misuse, and mental health outreach
workers. With successful precedents in other regions, shared learning
opportunities exist. The Outreach Psychotherapist will provide training to
partner outreach workers, enhancing their grasp of clients' psychological
issues to foster more effective interactions. The overarching goal is to guide
clients towards self-understanding, responsibility, and constructive
relationships, thereby aiding their transition from outreach work to
established treatment programs. Regular clinical supervision and collaboration
with the local Mental Health Trust's Lead Psychologist ensure a trauma-informed
approach and seamless client progression into mainstream treatment programs.
Main duties of the job
To
provide therapeutic interventions for clients who are homeless or vulnerably
housed and who have complex needs
Provide
clients with trauma-informed approaches, including psychological and
therapeutic support
To
hold a caseload of clients, working flexibly to meet their needs
Actively
promote the service and trauma informed practices to external agencies and
organisations and to attend Multiple Disciplinary Team Meetings (MDTs) for
clients on the psychotherapy caseload
Work
in partnership with other agencies and organisations involved in supporting
clients to stabilise their mental health and be supported to access and remain
in accommodation
Maintain constructive relationships with a
broad range of internal and external stakeholders including, GP surgeries and
other key stakeholders and partner organisations
Attend and actively participate in team,
service and wider multi-disciplinary team meetings as required
Create and maintain documentation as required

Supporting the service by providing strong
governance processes through maintaining electronic files and storing
associated documents within these

Peterborough. This role will work directly with a GP specialising in Mental

new Primary Care Rough Sleeper Outreach team.
The role will provide psychotherapeutic
interventions to people rough sleeping or at risk of homelessness who have been
referred to the service. Clients referred to the service will be rough sleepers
with moderate mental health difficulties who also have problematic and
interrelated health and social care needs, including substance misuse. These
interventions could be delivered in a variety of settings including on the
street on an outreach shift or attending a clients accommodation as required.
This new team is part of an integrated model of care
for Rough Sleepers including housing, substance misuse and mental health
outreach workers. The innovative outreach approach has been successfully
introduced in other areas of the eastern region and so there are opportunities
for shared learning with other areas.
The Outreach Psychotherapist will provide partner
outreach workers such as housing support workers and substance misuse staff
with regular training from the psychotherapist to help them understand the psychological
difficulties their clients experience and how to work effectively with them.
The aim of this training is to increase the number of staff in Peterborough
able to offer psychologically informed guidance to clients and, as a result,
help clients who often behave chaotically to gain an understanding of their
behaviour, take responsibility for themselves, and develop negotiated, positive
relationships.

external monthly clinical supervision and work closely with the Lead
Psychologist in the local Mental Health Trust, Cambridgeshire and Peterborough
Foundation Trust (CPFT), to develop a trauma informed psychological program and
ensure clients transition smoothly from the initial work of the outreach team
to stabilise clients into mainstream treatment programmes offered by CPFT.
