We have an exciting opportunity for a Partnership Lead to join our Tower Hamlets Service to lead activity which supports the organisation to understand and engage with these processes.

You will lead and sustain a service integration strategy and have a key role in developing greater collaboration between CGL services and external partners within integrated care systems and combatting drugs partnerships. This will accelerate progress in how our services are meeting health and social care (H&SC) challenges and responding to the Drug Strategy. The role will help local teams to understand system challenges, focus on what works and improve the care and treatment experience of people using services.

Main duties of the job

Purpose of role: Changes to the Health and Social Care system and Drug Strategy developments are moving at pace.

The purpose of the role is to lead activity which supports the organisation to understand and engage with these processes. You will lead and sustain a service integration strategy and have a key role in developing greater collaboration between CGL services and external partners within integrated care systems and combatting drugs partnerships. This will accelerate progress in how our services are meeting health and social care (H&SC) challenges and responding to the Drug Strategy. The role will help local teams to understand system challenges, focus on what works and improve the care and treatment experience of people using services.

You will provide information, advice and guidance to service-based leaders to enable services to position themselves to best serve the local system and communities. You will contribute by supporting CGL services to explore and evidence how they closely integrate with criminal justice and H&SC providers. The aim being to join up care locally around peoples needs, reduce drug and alcohol harms and contribute to a reduction of health inequalities.

Job responsibilities

Key Activities:

Map, monitor and understand the local Integrated Care System (ICS) and relatedactivity.

Map, monitor and understand the local Combating Drug Partnership (CDP) strategyand outcomes framework.

Lead and coordinate data analysis to feed into ICS and CDP developments

Take on project management of any partnership developments includingpartnership projects funded through additional funding grants

Build and sustain healthy relationships with local partners and respond in a solution-focused way to challenges

Understand local variation in ICS/CDP developments and internal systems to providelocal staff/teams with tailored advice and guidance, as needed, and be a single point-of-contact regarding ICS/CDP.

Develop relationships with CDP and sub-groups, Primary Care Networks including

Clinical Directors, local place-based partnerships, Mental Health Transformation

Teams, Acute trusts and VCSE alliance.

Upskill staff and teams by providing information, advice, and guidance, thatunpacks the complexities and enables services to integrate across criminal justiceand the health and social care system.

Monitor, capture and report on all OHID grant funded activity and provide anyoverview update to Service(s) Manager and commissioner reporting.

Apply a strength-based and system-focussed approach to working, sharing bestpractice and cascading any learning from positive service activity.

Educate local leaders about the value of the service on a human level and torecognise the purpose and benefits of integrated working.

Identify opportunities and embed partnership development events across the service to support specific learning/activity that will contribute to reducing drug and alcohol related harms and address local health inequalities.

Provide guidance on Service Level Agreements, Information Sharing Protocols andJoint Working Practices with key partners across the service, liaising with nationalorganisational leads for support as required.

Work with local management teams to embed neighbourhood and place levelpractice to further build positive working relationships based on CGLs core values.

Communicate with national organisational leads regarding ICS and CDPdevelopments and cascade to service.

Promote the principles of integration and remove barriers to sharing resources,processes and practice with internal and external partners (e.g., sharing bestpractice across services, consent to records, MDT plans, co-location, continuity ofcare etc.)

Be part of the service leadership team contributing to priority setting, attendingrelevant internal and external meetings (monthly/bi-monthly/quarterly)

Line manage externally-facing roles as required.

Be a positive spokesperson for the work CGL and partners do in making real,practical improvements to our service users lives.
