Change Grow Live is one of the UKs leading health and social care charities and the Substance Misuse Nurse role is a varied one. You will be supporting people with multi-faceted issues related to the impact of substance use on their physical and mental health.
As a Substance Misuse Worker working within local GP surgeiesyou will provide support to people that have been identified as requiring help with addressing their alcohol and/or substance misuse. You will deliver a range or interventions and provide a package of holistic support to help build resilience and improve peoples ability to achieve their goals and build recovery.
Idealy for one of these roles we are looking for someone who can speak fluently in one of the following: languages Hindi, Gujarati, Urdu or Punjabi.
This is an excellent opportunity for someone who has experience in a similar field looking to take the next step in their career.
Essential Car user due to these roles being set in the community we require you to hold a driving licence with access to a car.
Main duties of the job
The Recovery Worker will work as part of the RISE team but will be based in GP surgeries accross the borough of Ealing.This post sits within the wider community engagement team at RISE, which provides a range of peripatetic services within the Borough or Ealing.You will provide support to people that have been identified as requiring help with addressing their alcohol and/or substance misuse.You will deliver a range or interventions and provide a package of holistic support to help build resilience and improve peoples ability to achieve their goals and build recovery.
You will provide psycho-social interventions within the primary care setting to those who are not currently accessing the core RISE hubs and workcollaboratively with the professionals in Primary Care settings to provide support to those with alcohol and/or substances misuse issues. You will also be working with the professionals in Primary Care and RISE to ensure the people we support have regular health checks including BBVs and other relevant screenings where required

As directed by the Senior / Team Leader, the post-holder will be an integral member of a multidisciplinary team offering specialist clinical interventions individuals accessing our services.

The GP based Substance Misuse Recovery Worker will work as part of the RISE team. You will provide support to people that have been identified as requiring help with addressing their alcohol and/or substance misuse. You will deliver a range or interventions and provide a package of holistic support to help build resilience and improve peoples ability to achieve their goals and build recovery.
They will work towards:
- Supporting service users from point of entry into the service and through their treatment/recovery journey
- Providing psycho-social interventions within the primary care setting to those who are not currently accessing the core RISE hubs.
- Working collaboratively with the professionals in Primary Care settings to provide support to those with alcohol and/or substances misuse issues
- Working with the professionals in Primary Care and RISE to ensure the people we support have regular health checks including BBVs and other relevant screenings where required
- Promoting carer, service user and community involvement
- Reducing drug and alcohol related harm to service users and the wider community
- Providing screening, assessment, psychosocial interventions, recovery planning and onward referral
