Job summary

Bems are currently looking to appoint 2 x 0.8WTE (30 hrs p/week) Health Inequalities & Population Health Management Facilitators, to become part of a new network team across B&NES.

This is an exciting new position that would suit an experienced nurse, healthcare or public health professional.

This role has been designed to help support the 6 Primary Care Networks within B&NES, to drive change on health inequalities. Primary Care Teams have always played a crucial role in improving the health of their local communities, providing holistic care and serving the needs of patients. PCNs are now building on these strengths in order to provide more population focussed, proactive, preventative and joined-up care.

This is a fixed term contract for 24 months and successful applicants would have access to NHS Pension scheme.

Main duties of the job

You will work with B&NES PCNs to bring together their frontline knowledge of their communities, with supporting population level data and intelligence on health inequalities. You will support PCNs to use this information to do things differently for their patients with a view to reducing health inequalities.

You will be supported to do this through working closely with the new B&NES Health Inequalities network and local and BSW intelligence teams.

- Supporting PCNs to deliver on the National Core 20+5 targets for adults &children.

- Working with practices and the voluntary and community sector to understand any additional health inequalities affecting their local populations.

- Working to support improvements in collection and accuracy of data where this may be required e.g. recording of ethnicity data.

- Working to embed Population Health Management as routine practice within primary care in B&NES.

- Working with PCN Health Inequalities leads, practice clinical and management staff to build capacity and capability around Health Inequalities and PHM across PCNs.

- Linking with the B&NES Health Inequalities network to coordinate activity across B&NES

About us

B&NES Enhanced Medical Services (BEMS) is a not for profit GP Federation, whose membership consists of all 22 GP Practices in B&NES.

We work very closely with the Practices and Primary Care Networks (PCNs) to provide clinical services at scale, as well as employing clinical staff under the Additional Roles Reimbursement Scheme (ARRS). This includes Pharmacists, Pharmacy Technicians, Nurse Associates, Physician Associates and First Contact Physiotherapists.

We also work closely with the local Training Hub to provide training and development opportunities and to support student placements in Primary care.

We have been rated as outstanding in the well led domain by the Care Quality Commission (CQC).

Key areas of work will include:

- Supporting PCNs to deliver on the National Core 20+5 targets for adults & children

- Working with practices and the voluntary and community sector to understand any additional health inequalities affecting their local populations.

- Working to support improvements in collection and accuracy of data where this may be required e.g. recording of ethnicity data.

- Working to embed Population Health Management as routine practice within primary care in B&NES.

- Working with PCN Health Inequalities leads, practice clinical and management staff to build capacity and capability around Health Inequalities and PHM across PCNs.

- Linking with the B&NES Health Inequalities network to coordinate activity across B&NES

Data analysis

- Work with the Business Intelligence team at the Integrated Care System and local analysts working with public health in B&NES Local Authority to produce easy to interpret reports for PCN/practices to better understand health inequalities at PCN and practice level.

- Work with Business intelligence teams in the ICS and in B&NES LA to understand how PCNs and practices are performing against CORE20+5 expectations.

- Work with clinical and practice staff to interrogate the data produced and to understand where there are areas where PCN or practice level activity could reduce health inequalities.

- Work with PCNs to identify cohorts of patients where new interventions could be targeted to reduce health inequalities.

- Support PCNs in agreeing measurable metrics to capture the impact of any interventions.

Project/programme management

- Once key issues are identified for PCNs, work to agree evidence-based interventions that will help address health inequalities.

- Research what interventions have been successful elsewhere in the country to help stimulate ideas.

- Help facilitate interventions that may be carried out by practice staff working differently within the practices. This could include GPs, practice nurses, pharmacists, first contact physiotherapists or social prescribing link workers and care co-ordinators.

- Write simple business cases for interventions that may involve other health partners working differently. The post holder will need to advise the ICA/ADOG for them to take this work forward.

- Utilise a quality improvement approach to these interventions.

- Facilitate closer working with social prescribing link workers and care co-ordinators to agree interventions that work closely with the third sector on issues relating to the wider determinants of health. Work closely with the Community Wellbeing Hub.

- Utilise the opportunity to plan, do, study and act for each of these interventions.

- Write up mini audits for each of these interventions.

- Share the results of these audits with the other PCNs in B&NES and BSW.

- Help support the on-going evaluation of this way of working.

- Primary Care Networks have a number of Directly Enhanced Services (DES) that they are expected to work towards. The post holder will support delivery of these requirements through their work. These DES requirements include:

o Tackling Neighbourhood Health Inequalities

o Personalised Care

o Anticipatory Care

o CVD prevention and diagnosis

o Early Cancer Diagnosis
