Job summary

Full-time and part-time Senior Clinical Practitioner £47,500 to £52,763, yearly salary increments, NHS Pension provider, rate enhancements for out of hours working.

We have fantastic opportunities for Senior Clinical Practitioners to undertake telephone triage based in our OOH Clinical Hub in Denton, and face to face triage/see and treat at our Walk-in Service in Manchester City Centre.

Our City Health Centre Walk-in Service is situated above Boots on Market Street, which offers a great selection of cafes, restaurants and shops with excellent transport links. Our Walk In Centre is open 08:00 to 20:00, 7 days per week and our Clinical Hub is open 24 hours a day, 7 days a week.

As a Senior Clinical Practitioner, you will work in a team who has a real commitment to continuous improvement, shared learning and best practice. We are passionate about providing patients with the highest possible quality care.

This is a dual-site role and working across both locations is essential. No experience of telephone triage is required as full training will be given.

Main duties of the job

  • Holistically assess and clinically examine patients who fall within your scope of competence, through history taking, physical examination and appropriate use of tools to confirm diagnosis. Plan, implement and evaluate an effective management plan for the patient with support from senior colleagues.
  • You must only prescribe within your scope of practice and comply with the NMP governance registration processes. If you are not a NMP, you will work towards this qualification.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Maintain effective communication with senior clinical colleagues, the wider team, the organisation and with external stakeholders.
  • Understand own role and scope and identify how this may develop over time.
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  • In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
  • Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice.

Clinical Practice

  • Assess, order and interpret investigations, undertake practical procedures, diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological, psychological and social perspective, and plan clinical care accordingly.
  • Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.

  • Anticipate barriers to communication and take action to improve communication.
  • Maintain effective communication within the organisation and with external stakeholders.

  • With relevant training, authorisation and experience, practitioners may be trained to use a set of specific patient group directions (PGD's). These are applicable only to nominated, authorised and competent practitioners who have undertaken training in this sphere of practice.
  • You must maintain your knowledge and skills with regards to prescribing and administration of drugs, keeping up to date with any changes, alerts, recalls and other reasonable professional knowledge.
