City Health Centre, part of gtd healthcare, is a practice located in Manchester City Centre. An excellent and rewarding opportunity has arisen to join the Urgent Primary Care Hub as an Advanced Clinical Practitioner.
The successful Advanced Clinical Practitioner will join a team of highly skilled and compassionate staff, committed to providing excellent and safe patient-centred care for all who access our services.
The Urgent Primary Care Hub is open 8.00am - 8.00pm and 7 days per week, we have flexibility of full-time, part-time, midweek or weekend working hours available.
We are situated above Boots on Market Street, which offers a great selection of cafes, restaurants and shops with excellent transport links.
The successful candidate will be qualified to MSc level in Advanced Clinical Practice and hold a prescribing qualification. The candidate can be from a Nurse, Pharmacist and AHP background. Within the role it is required that you are a competent, safe practitioner with primary or urgent care experience.
We welcome applications from both newly qualified and experienced Advanced Clinical Practitioners, support and development via our gtd training academy will be provided.
Main duties of the job
- Using a electronic clinical system, the successful candidate will consult via telephone, video or face to face, according to patient needs.
- Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
- Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Refer and discharge safely ensuring that safety-netting advice is offered.
- Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
- The successful candidate will possess leadership qualities and provide support to the wider team, acting as a positive role model and proactively identify areas for quality improvement.
- In addition, you will assist to further improve standards of care and, achieve quality targets as well as KLOE objectives, as outlined by CQC.
