Job summary

gtd healthcare are one of the largest urgent and primary care healthcare providers in North West England, we are proud to have a member practice of the Oldham Central Primary Care Network (PCN) and work in collaboration within the PCN to support the healthcare needs of the Oldham population.

The Oldham Central PCN are currently recruiting for a Advanced Nurse Practitioner. The Oldham Central PCN is a network of 11 GP Practices lying within the Oldham Integrated Care System. Oldham South PCN services a population of 72,000 patients.

The Advanced Nurse Practitioner will work across the PCN to support the delivery of the Oldham Central Population Health Management Programme. This programme is focussed on providing more proactive support to at-risk children and families.

The role is highly relationship-driven, and will involve building excellent relationships across our PCN and the wider health and care system in Central Oldham through a new integrated team as well as building strong, trusted relationships with the children and families this model seeks to support.

More specifically, the post-holder will work within the PCN team and across an integrated neighbourhood team of different health and care professionals involved in the care of this group of patients to coordinate their care plans. As mentioned, the role will involve patient facing care, and the post holder will be responsible for providing support directly to patients and their families.

Main duties of the job

  • Provide clinical leadership and supervision to the PHM care coordinator and other staff involved in the delivery of the Oldham Central model for at-risk children and families.
  • Conduct holistic health and care assessments to assess the different needs of patients and families identified as in need of more intensive support by the PHM care coordinator.
  • Co-develop shared care plans with families to address needs identified in the assessment, and support families to implement these, with support from a wide range of health and care professionals including the NCA, childrens social care and VCSE.
  • Refer patients to other health, social care and VCSE services.
  • Provide expert professional advice to patients, carers and colleagues.
  • Undertakes Clinical Nursing Practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.
  • Evaluate patients response to health care provision and effectiveness of care.
  • Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
  • Support the PCN on implementation of this new model of care by providing updates & feedback on the model and helping adapt and improve it going forward to best meet the needs of our patients.
  • Identify where there may be health inequalities and provide feedback on where engagement could be enhanced.

  • Can recognise personal limitations and knows when to seek support from colleague(s).
  • Follow through with service users and others involved to ensure all services and care arrangements are in place.
  • Develops an in-depth knowledge of local health and care infrastructure and knows how and when to enable people to access support and services that are right for them.
