Job summary
gtd healthcare is one of the largest NHS commissioned Primary and Urgent care providers in North West England. An exciting opportunity has arisen for an Care Coordinator within the Specialist Services provision.
As a Care Coordinator, you will be the first point of contact for health care professionals, patients and patients relatives accessing onsite Primary Care satellite clinic appointments at bridging and asylum seeker accommodation locations across Manchester.
Under the direction of the Operations Officer you will register patients onto the relevant clinical system and not only make direct GP/Practice Nurse appointments but also direct them to the most appropriate service via referrals.
You will also work directly with other services that reside onsite to form part of a multidisciplinary team to deliver care and support to refugees and asylum seekers across Manchester.

Main duties of the job
- To deliver a high-quality care co-ordination service to patients and health care professionals.
- To accurately record on a range of clinical systems all relevant data for each contact and direct appropriately.
- Where demographics are incomplete, complete all necessary checks e.g. NHS Spine to ensure most up-to-date records are available.
- Undertake the booking of appointments for the onsite clinics daily and future bookings.
- Provide a first-class reception service, ensuring a professional meet and greet, efficient registration and onward referral to the relevant area/service.
- Liaise with the Operations Officer, any operational issues affecting the daily delivery of service.
- Follow the failed encounter procedure for all patients that did not attend (DNA) for a face-to-face assessment.
- Coordinate the arrangement of any follow-up care or referrals as directed by the Operations Officer or onsite Clinician.
- Liaise with the multi-disciplinary team to ensure the smooth running of the service.
- Fully comply with the Complaints Policy ensuring all informal and formal complaints and incidents are documented and the process is explained to patients.
- Responsible for undertaking administration duties within the service including sending referrals on behalf of the clinical team, supporting Hospital appointment attendance and completing tasks.
