Job summary

Indigo Gender Service is a collaborative, innovative model of care between gtd healthcare, primary care and the LGBT Foundation for the NHS. We have an exciting opportunity for a Senior Clinical Practitioner to come onboard.

Indigo is a pioneering, localised and patient centred model that provides gender affirming healthcare for trans* and non-binary patients across Greater Manchester. We particularly welcome applicants from people of colour, trans and/or older (50+) as colleagues from these backgrounds are currently underrepresented in our organisation.

As a Senior Clinical Practitioner you will support GPs and other health and social care providers across Greater Manchester as a learning and educational resource, improving knowledge of trans and non-binary health issues. The clinician will act as a positive role model supporting junior staff whist working flexibly provide safe, clinical and expert care. They will participate in and lead change for the benefit of patients and/or the organisation and be a key member of the senior clinical team.

The successful candidate willbe expected to be flexible regarding travel and hours to suit the organisational demands.

Main duties of the job

Your key role will be to support relevant medical and associated administrative duties in the assessment, diagnosis and treatment and follow up of trans, non-binary and gender variant people. You will work closely with the Indigo GPs and other clinicians to deliver on the core responsibilities of the service specification. You will have relevant interest in the provision of gender identity healthcare within a primary care setting. This may include but not be limited to:

  • Undertaking first assessments for gender incongruence.
  • Follow up and review appointments.
  • Reviewing blood results related to gender affirming hormone treatment.
  • Updating care plans.
  • Taking bloods and giving injections.
  • Attending MDT and other meetings.
  • Signposting and support.
  • Discharge appointments.
  • Responding to clinical questions and queries from primary care.
  • Providing education and training.

You will be motivated to undertake self-directed learning as well as further RCP training via the professional certificates for gender identity professionals, alongside gaining clinical experience of trans healthcare within the Multidisciplinary Team setting of Indigo Gender Service (funded by NHSE).

You will be vital asset to the clinical team providing a link between the care navigators and GPs. Providing clinical advice and support to Indigos patients using a blended approach of face to face, telephone and video consultations.

Clinical Practice

  • Provide high quality and current evidence based medical care to the patients of the Indigo Gender Service.
  • Actively participate in the development of the service and relationships with existing services to provide a holistic, community focused service that meets the needs of a diverse group of patients.
  • Holistically assess patients to help create an individualised and specific treatment plan.
  • Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • You will be skilled in particular clinical areas that complement the General Practitioners, to include contraception, fertility, screening etc.
  • You will develop your skills and competence alongside other clinicians.
  • To support a level of continuity of care within a service that helps reassure patients and provide a degree of clinical input.
  • To support the longer term follow up of patients that are already established on a medical treatment pathway that have not yet been discharged back to the care of their GP.

Communication

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Anticipate barriers to communication and take action to improve communication.
  • Maintain effective communication within the organisation and with external stakeholders.
  • Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Non-Medical Prescribing

  • If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with NMP governance registration processes.
  • You must maintain your knowledge and skills and prescribe within your clinical competencies.
  • The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

Delivering a Quality Service

  • Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
