Job summary

An exciting opportunity has arisen for an enthusiastic and dedicated Clinical Pharmacist to join our practice team and provide care for our patients at Gordon Street Medical Practiceproviding care for 4800 patients.

The successful Clinical Pharmacist will have GP experience and be keen to be part of a larger organisation as well as a smaller practice team. Our practice populations are diverse and multi-cultural with a strong community spirit. You will be part of a busy and proactive team working alongside clinical and non-clinical members of the team and the wider organisation.

The role is pivotal to improving the quality of care, leading innovation and being aware of operational efficiencies. Main duties of the job

  • Within your professional boundaries you will provide advice, care and medicines management for urgent and/ or primary care patients either via telephone/ video or face to face.
  • Supporting the delivery of policy, procedures and leadership as required. This may involve the management of long term conditions and diagnosed or undiagnosed undifferentiated illness if within your competence.
  • Acting as a positive role model supporting junior staff whist offering the ability to work flexibly between urgent care and primary care to provide safe, clinical and expert care.
  • Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
  • Provide support to other clinicians with regards to prescription and medication queries/ repeat prescriptions, and will demonstrate clinical reasoning, critical thinking.
  • Ensure that services integrate with community and hospital pharmacy to help improve patient outcomes, ensure better access to healthcare and help manage workload. There will be an element of clinical audits and be expected to feedback the results and implement changes in conjunction with the practice/ service team.
  • Participate in and lead change for the benefit of patients and/or the organisation and be a key member of the senior clinical team being proactive in providing safer prescribing, providing expertise in clinical medicines advice, while addressing both public and social care needs of patients in their care.
