gtd healthcare is one of the largest NHS commissioned Primary and Urgent care providers in North West England. We are currently expanding our bank of Practice Nurses within gtd. If you wish to work flexibly in the evenings or weekends or day time core hours then we would love to hear from you.
gtd healthcare provide practice nursing services to patients across many of our localities including:
Oldham, Tameside & Glossop
Manchester, Derbyshire. Practice Nurse services are deliver from our GP practices, Extended Access Services, (EAS) and specialist services which includes caring for asylum seekers and refugees.
We are looking for enthusiastic and experienced Practice Nurses to join our team.
Many of our services re located in some of the most deprived areas. This provides an opportunity for our team of Practice Nurses to make a positive difference to some of the most vulnerable communities that we service. Main duties of the job
- Assess, diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with undifferentiated diagnosis.
- Plan, implement and evaluate an effective management plan for the patient care service.
- Diagnose and manage acute episodes of care integrating both drug- and non-drug-based treatment methods into a management plan.
- Support patients and their families to adopt health promotion strategies that promote healthy lifestyles.
- Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
- Participate in the maintenance of quality governance systems and processes across the organisation and its activities.
- Utilise the audit cycle as a means of evaluating the quality of the work and implementing improvements where required.
- In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
- Any other duties, as agreed with the line manager to meet the needs of the organisation. This may include travel to other sites within the organisation.
