Job summary
Guide Bridge Medical Practice, part of gtd healthcare, is looking for an enthusiastic and dedicated Clinical Pharmacist to join our small team providing care for our 5000 patients at Guide Bridge Medical Practice. We're looking for a Clinical Pharmacist with GP Practice experience who is keen to be part of a larger organisation as well as a smaller practice team.

Main duties of the job
Within your professional boundaries you will provide advice, care and medicines management for urgent and/ or primary care patients either via telephone/ video or face to face.
Your role will include supporting the delivery of policy, procedures and leadership as required. This may involve the management of long term conditions and diagnosed or undiagnosed undifferentiated illness if within your competence.
The clinician will act as a positive role model supporting junior staff whist offering the ability to work flexibly between urgent care and primary care to provide safe, clinical and expert care.
You will be expected to be able to undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide support to other clinicians with regards to prescription and medication queries/ repeat prescriptions, and will demonstrate clinical reasoning, critical thinking
The post holder will ensure that services integrate with community and hospital pharmacy to help improve patient outcomes, ensure better access to healthcare and help manage workload. There will be an element of clinical audits and be expected to feedback the results and implement changes in conjunction with the practice/ service team.
