An opportunity has arisen to become part of Greenwich Healths Live Well Service team.
This role is a varied one and involves working from our main Greenwich Health office, as well as at our Live Well centres across the borough. You would be taking on the administrative and Health Care Assistant tasks required to fulfil our commitment to deliver all our commissioned services for the Royal Borough of Greenwich in NHS Health Checks, Stop Smoking Services and supporting our Contraception and Sexual Health Services.
We are looking for a friendly, committed person who has great communication skills.

Main duties of the job
Deliver NHS Health Checks, smoking cessation interventions,
LARC clinic assistance across Greenwich within the Live Well Centres.
Carry out the NHS Health Check, smoking cessation
clinics, and LARC clinics following protocols and clinical guidelines.
Providing telephone support for patients within
the call centre including managing incoming calls, dealing with referrals
from our GP practices, triaging patients, providing pre-appointment information and booking confirmations.
Participating in regular meeting with the
service lead to support the development of the role and the service.
Deliver services with a patient focused approach
to help individuals identify how their behaviour might affect their health and
wellbeing.
Deliver services in line with evidence-based
practices.
Motivate and support individuals to make
lifestyle changes and provide appropriate onward referrals.
Prepare, organise and collate own paperwork,
equipment and resources ready for delivery of services.
Keep accurate and factual patient records in
line with Information Governance, Data Protection Act and Greenwich Health
policies.
Complete timely and accurate data entry onto the
company clinical system.
Work alongside the service lead to ensure
administrative tasks are undertaken according to service protocol.

Job responsibilities
The post holder will be required to work within a team providing lifestyle and behaviour change interventions through NHS Health Checks, smoking cessation advice and assistance to our clinicians providing surgical contraception services (training will be provided). These services are provided within four Live Well Centre’s across the borough.
In addition to providing these services, the post holder will also operate within our call centre, handling patient queries, managing appointments, and triaging patients calls.
The NHS Health Check is a behaviour change intervention to access cardiovascular risk. Carrying out a Health Check involves measuring blood pressure and pulse, taking a near-patient blood sample to measure cholesterol, measuring height, weight and BMI and asking questions about lifestyle.
Smoking cessation is a behavioural change service designed to work with patients on a journey to quit smoking. The successful candidate will be expected to work with colleagues and stakeholders to deliver 1-to-1 programmes to patients providing advice, treatment and motivational support and encouraging behaviour change.
Our Live Well Centres offer Long-Acting Reversible Contraception (LARC) services. The post holder will support this service by assisting the clinician, supporting the patient, and processing data and record keeping. The role will also involve supporting patients through managing the referral list, triaging, and managing bookings through working in the call centre.
The call centre is based within our head office and is an integral part of the role. The post holder will manage incoming calls, deal with referrals from our GP practices, provide triage of patients, provide pre-appointment information to patients and booking confirmations. There will also be virtual smoking appointments from within this setting to provide different access options for patients.
Training
· Partake in appropriate training for each of the service areas to ensure high standards of care.
· Complete all mandatory company training, including annual update / refresher training.
· Take an active part in developing own knowledge and skills, particularly the key skills and competencies required to carry out the NHS Health Checks and smoking cessation.
· Keep abreast of changes within the health industry via the reading of literature, attending seminars and continuing professional development
