Woodsley & Holt PCN would like to recruit a Podiatrist to work within our Primary Care Network (PCN) multidisciplinary healthcare team.

Our ambition is to improve management of patients suffering with various foot conditions and other at-risk groups that have limited access to our community podiatry service.

Main duties of the job

To work as part of the PCN which comprises of 10 local practices with a total population of around 100,000 registered patients. This role will work closely with other PCN roles: Pharmacists, Physiotherapists, Dietician and Social Prescriber in addition to the core practice teams.

You will assess, investigate, manage foot conditions and refer patients who have been directed to your service by a primary healthcare worker or directly by reception where appropriate. You will be an experienced practitioner who is comfortable working independently as well as part of a wider multidisciplinary team. You will be supported by the clinical team at each GP surgery and work with the wider clinical team to deliver holistic care to our patients. You will be able to assess and assist in adapting the service in response to local population needs.

Woodsley & Holt Pak PCN covers the following areas: Woodhouse, Kirkstall, Burley, Hyde Park, Headingley, Cookridge and Horsforth areas of Leeds, although it is expected the post holders will develop and maintain close links across the city.

Work as part of a PCNs multi-disciplinary team to clinically assess, treat, and manage a caseload of patients of all ages with lower limb conditions and foot pathologies, using their expert knowledge of podiatry for specific conditions and topics

Utilise and provide guidance to patients on equipment such as surgical instruments, dressings, treatment tables and orthotics

Work towards developing the service to include the options to prescribe, produce, and fit orthotics and other aids and appliances

Provide specialist treatment and support for high-risk patient groups such as the elderly and those with increased risk of amputation

Work towards developing the service to include supporting patients through the use of therapeutic and surgical techniques to treat foot and lower leg issues e.g. carrying out nail and soft tissue surgery using local anaesthetic

Deliver foot health education to patients

Implement all aspects of effective clinical governance for their own practice, including undertaking regular audit and evaluation, supervision and training

Liaise with PCN multi-disciplinary team, community and secondary care staff, and named clinicians to arrange further investigations and onward referrals

Communicate outcomes and integrate findings into their own and wider service practice and pathway development

Develop, implement and evaluate a seamless podiatry support service across the PCN, working with community and secondary care where appropriate, and aimed at continuously improving standards of patient care and wider multi-disciplinary team working.

To maintain Registration with the Health and Care Professions Council HCPC, and maintain professional standards of treatment and conduct

Undertake continued professional development to understand the mechanics of the body in order to preserve, restore, and develop movement for patients;

Provide leadership and support on podiatry clinical service development across the PCN, alongside learning opportunities for the whole multidisciplinary team within primary care;

Provide education and specialist expertise to PCN staff, raising awareness of good practice in good foot health;

Ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate PCN and local population objectives;

Undertake a range of administrative tasks such as ensuring stock levels are maintained and securely stored, and equipment is kept in good working order.

To encourage multi-disciplinary working between all professional disciplines, developing a liaison culture.

To regularly attend locality multidisciplinary meetings in order to share information on local needs and initiatives and support the development of the Podiatry service.

To ensure all duties are carried out to the highest standard and in accordance with current quality initiatives within local areas and to meet departmental activity targets.

Actively participate in, and show personal commitment to, structured Continuing Professional Development [CPD] and a variety of learning opportunities outside structured CPD, in order to develop new skills, and maintain and update existing skills.
