Job Purpose

The Care Homes Social Prescribing Link Worker will be an integrated member of a broad primary care team at Seacroft PCN. They will work within The Care Homes Team supporting residents across three practices Park Edge Practice, Oakwood Lane Medical Practice and Windmill Health Centre

Main duties of the job

Main Duties and Responsibilities: the below are not exhaustive and may include other reasonable duties and responsibilities

The post holder will support individuals to build independence and personal resilience, enabling self management of their health and wellbeing. Key to the role will be the ability to build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively and overcoming barriers.

Social Prescribing Link Workers will build and develop relationships with a wide range of services and activities in order to address social and health care needs, signposting and offering support to individuals to participate and benefit from community-based activity.

Blood tests requested on ICE following direction from a clinician.

Phlebotomy in 3 out of the 5 care homes covered by Oakwood Lane and Park Edge Practice

Blood form printing and delivering to care homes.

Blood pressure taking following a request.

Takes the lead for proactively organising the data collection for annual health monitoring of all care home residents.

About us

For proactive care undertakes the role of collecting the data, inputting data and where appropriate educating patients as part of the part 1 section for the Year of Care process. This could include:

    • Hypertension monitoring
    • Weight monitoring
    • Proving lifestyle advice
    • Taking pulses and being able to identify a irregular pulse
    • COPD monitoring- including inhaler technique, recording MRC
    • Asthma monitoring, inhaler technique, completing the Asthma Control Test questions.
    • Diabetes foot checks
    • Recording smoking, alcohol intake and drug taking behaviour
  • Under takes the phlebotomy required as part of proactive care and any which arise acutely
  • Escalate clinical information which may indicate any long term condition is not controlled
  • Within competence, can review a patients long term health monitoring requirements and seek advice where they feel the monitoring might not be appropriate
  • Undertake new resident reviews to gather data as per new resident form which is under development and input this data
