Job summary
Place of work: GP practices across the LS25/26 PCN will include visits to patients homes
Areas of intervention for our patients with frailty are vulnerable or housebound
- To visit and support patients at home to assess whether there are any unmet health or care needs.
Help those who want to remain at home be as independent as possible.
To initiate holistic assessments.
- Where unmet needs are identified to complete relevant referrals/care coordination into the PCN and community services.
- To attend and contribute to MDT meetings.
Support the assessment of mental health problems.
To use clinical skills such as venepuncture and taking clinical observations.
To hold in house clinics if required
To use the available resources within the community to meet holistic needs

Job Description
Assessment
Work as part of the multidisciplinary team making home visit or working in clinics across a population of patients who are mostly frail vulnerable or house bound
Carry out a holistic, person-centred assessment which looks at the health and social needs of the person, conducted in partnership with the individual person, their family and carers. The assessment will be conducted, in the persons own home or in clinic to take account of environmental factors and to fully understand the persons functional ability in a familiar setting.
Carry out agreed specific assessments including a basic falls risk assessment, mental health and dementia assessments training can be provided
Develop and agree care plans and if necessary, reviews for ongoing needs.
Support with information gathering; undertaking CCSP Part 1 as part of long-term condition management.

Since the formation of PCN in 2019, we have gone from a small team of existing practice staff to a large team of over 20, and this continues to grow. Our staff in all our practices and our PCN take pride in serving the community, and I would like to thank them for all their hard work and commitment.
